Status post circumcision, swelling on top and bottom opened up

Dr Reed, i wrote  you an email a couple of months ago regarding the circumcision i had done in which the skin on the bottom of the penis split open after operation. Dr. Reed i am not happy with the way it has healed plus the swelling around the top of the head does go away and to be honest with you i cant stand the sight of it. Needless to say my self- esteem is extremely low. i value yourt opinion as a specialist and i regret whole heartedly not seeing one to begin with and having this operation done by a surgeon that does not specialize in these types of operations. i look forward to meeting with you. Hi Frank,    Sometimes a separation like that will close like a zipper  and the result will be imperceptible versus if it had never  opened. You’ll have to take a wait and see attitude,  but they all will close. After 3 or 4 months following complete  wound healing;, if you are still not satisfied. perhaps you could  see me for a revision.

Phimosis and weighs 350 pounds

I am 67 yrs of age and weigh 350 lbs. I have phimosis and I am considering a circumcision. I have Medicare and AARP  and do not have other funds avaiable. I would like to get done as outpatient, no hospital stay. I don’t like the idea of spending a night in hospital. Would this be possible?   Sid

Good morning Sid,

We respectfully would decline to do your circumcision at our office based facility but would in a hospital setting and yes, most likely you needs would be best served to have you stay overnight.  However, we could request an early morning starting time, and most likely if you are stable, you could be discharged later (same day).   Accepting Medicare as the primary and Aetna as a secondary is no problem as long as their regulations are followed.

I know you would not get on a plane with a 5% likelihood of a major mechanical malfunction, why take chances with surgery?

We do ask that you stay for one, ideally 2 days at the Baltic Hotel (advanced tourist class) and I will see you there daily.

Tears and Bleeding with Sex

I have been experiencing deep cuts on and around my penis head during and after sex. I don’t know if my girlfriend is just that close built or is it because of the friction that accures during sex. Do you have payment plans for self-pay and if so how much will be required for the procedure to take plac? Thanks

Good morning Johens,

Thank you for your interest in what we do.  Healthy penile skin should not tear
under normal circumstances.  Chronically injured and somewhat devitalized
skin will.  Most likely your head is fine but the distal foreskin is not.

Yes, I would concur a circumcision would be beneficial.

We have performed over 5000 cosmetic circumcisions in the past 35 years
and usually do several cases every week.

Need a circumcision revision, from Australia

Greetings Dr. Reed and/or staff. Thankyou for your site/s. They have proven extremely informative. Hopefully I am not troubling you with this question – but there is no one in Melbourne who seems to specialise in this field… I had an adult circumcision earlier this year due to phimosis. I was hesitant to have the surgery done, but years of complications urged me otherwise. As I didn’t have insurance, I was assessed at the local hospital. I was never asked what kind of circumcision I was after, so it never dawned on me there were variations to the method. I was naive enough to think there was just one ’style’. The surgeon who performed the surgery did not inform me on the day of the outcome, so I went into the surgery innocent and goofishly optimistic. The surgeon performed a ‘low and loose’ circumcision – about half an inch below the glands. The problem I have is that the cut is very uneven, and I have what appears to be obvious suture marks/skin tunnels. The stitches were left in for 2-3 weeks. Kiernan

is your circumcision and we do it the way you want with respect
to 4 parameters.

Please don’t be like the patient who writes: “Dr. Reed I just got circumcised
two days ago and my result was not what i expected. I was wondering
when i recover can i get a consultation with you.” Photos sent
and received (unspoken comment “disaster zone”)

Even laying a finger on one side asymmetrically while holding the penis
can create distortion and an uneven circumcision line. As they say in
carpentry, measure twice, cut once.

Phalloplasty Length and Girth, Questions

 I am very interested in going forward with this procedure. 
 
Are the results permanent?
 
 Is there a mini um gain in length. I guess what I’m trying to say is if I did this can I guarantee myself ant least 1 in in length?  Is there a package price if you have all three done at the same time? I wouldn’t do just one i want both. I already have a big head well bigger then the shaft.!  Please let me know As soon as possible.

We hope you can relate to our photo gallery of befores and afters. Our fees are posted on our web-site.Patients who have lengthening usually leave for home the next day.  You could probably return to an office environment in 3 days. Or consultation on Thursday, surgery on Friday.  Monday, back to work.

 

 

 

 

From surgery anticipate a 3/8″  increase in length and with dedicated use of traction, 8 hours a day in divided sessions, anticipate a 1/8″ gain each 

month such that after 6 months you’ll have 3/8″ plus 6 x 1/8″ or 9/8″.  We say about an inch longer erection in 6 months.  Flaccid lengths will be corresponding longer

Whenever you stop using traction whatever you have gained is permanently yours. 

If you continue, more length gains will be accrued.
We do not do simultaneous surgery, such as any combination of length, girth of shaft, girth of glans, circumcision, or pubic liposuction.

Hardly a week goes by when some disillusioned patient done elsewhere calls us for a revision.  And we told you so.

Generally speaking the penis does not appreciate simultaneous surgery with a few exceptions.

AlloDerm is like particle board, when immersed in body fluids, it swells and swells for several days.  This spreading of the collagen fibers is helpful to permit ingrowth of a capillary bed nd new cells of your own making.

Early mobilization  is the great enemy of good graft ake.   Again plan on staying in your room, stocking up on food or sending out for food.  Good Italian, Greek and Chinese restaurants abound with delivery service only a telephone call away..  I’ve have been known to go shopping for some patients at Publix.  You won’t starve.

If you have AlloDerm surgery on Tuesday, you could be back in an office environment on the following Monday.

Wrinkly foreskin, what happens after adult circumcision

Penises and their foreskins come in all shapes and sizes.  The foreskin has 2 surfaces an inner or mucosal skin and an outer of epidermal skin.  The inner, in situations where there is not a lot of exposure to the outside world, may take on a wrinkled, smooth slightly moist glistening appearance,

A circumcision in its simplest form is really about removing a large portion of the foreskin, usually some from each side, and reconstituting the edges so when healed the skin does not lie on the head of the penis.

Still in that most always some inner skin remains what to do.  Needless to say, this has to be discussed with the patient beforehand.  More could be removed but what remains is still wrinkly.  One could remove a strip from the underside of the inner skin where a fine scar would be least seen. 

All in a days work, but conferencing beforehand with the patient is most important.

Thank you for your interest in what we do.  BXO, now called by the preferred term lichen sclerosis, does need to be treated because of its slow premalignant potential Several reports have suggested the development of squamous cell carcinoma.  More commonly there is a slow spread and  tendency to grow into the urethra and producing scars and strictures there.  At times, the foreskin in some areas is densely adherent to the glans (head of the penis).  We see this a few times a year and have performed very possibly more adult cosmetic circumcisions in our office than anywhere else in the United States.  Well over 5000 thus far.  Usually several every week.

Please do not be like the patients we hear from every so often who send in photos 2 days after their procedure with obvious hideous result, and ask “what do I do now.”  Needless to say, a fair amount of circumcision revisions are also done here.

This is your circumcision and we do it the way you want.  There are four important parameters.  A hand held mirror is available in the operating room so you can be assured that each detail is done in accordance to your wishes.
