More on penile skin bridges and tunnels and bumps and an overly tight circumcision

February 13, 2011

Dr. Reed,

Someone referred to you from the yahoo.com website. I was circumcised last April and my surgeon used dissolving stitches. There some raised bumps where the stitches were. There was tunnels but they’ve gone away.

I’ve been putting vitamin E on it cause I heard that helps heal scars. I just want the raised skin gone. Even if its a little discolored, just no raised skin. I didn’t start treating the scars till six months after surgery(roughly two months ago).

Someone told me it can take up to 18 months to completely heal. I just wanted to know if this is permanent or if it should heal or if there’s anything more I can do?

If you can’t help me I understand I just don’t have health coverage anymore so I can’t ask my doctor. I got the circumcision done cause I was really insecure about it and the skin was WAY too tight.

Thanks in advance.

Donald

Hi Donald,

These raised bumps may be chronically inflamed areas that are the remnants of suture tunnels.  Best remedy is removal together with curettage or delicate knife and closing with ultra fine fast absorbing suture and keep the steri-strips on for 3 or more weeks.

There are also non-surgical ways to loosen your overly tight skin.  Will discuss during your consultation.

Please see our web-site http://penisdoctor.com/photo-circumcision.htm and look at photographic examples 11/1-3.  Been there, done that.

Have a restful Sunday,

Good afternoon Benson,

Taking your thoughtful questions in the order posed…

Seriously, I’ve wondered if it ever restricted the size when erect?.since the skin gets so tight??.of course only you would know this answer. ??

Probably yes, as men who have phimosis report that their glans gets bigger especially when erect.

I wonder if the ?Corona? is still intact and with proper ?form? being enclosed all these years. ??

Again based on our experience with bridges and phimosis I would think yes.  There may be some adhesions which will have to re-epithelialize over.   There are no iron clad assurances in medicine.

So you don’t see any issues with this repair?

No sir, I do not aside from the usual advisories for any incision.

Truly you are fostering a premalignant (that’s saying it kindly) situation, could even be exuding carcinogens during sexual relations.  I know you would not get on board a plane even if there was a 2%
chance of a serious mechanical malfunction.  Why take a chance with your glans?

Sincerely,

Harold M. Reed, M.D.

Preputial bridges covering corona yearning to breath free

Hey Doc ?  how about extensive

glanular-preputial skin bridges left over from a botched circumcision as an infant? Is this something you can correct?
I would not want to proceed unless I would be happy with the end result. I certainly would not want to do it if it left my penis looking abnormal.  What do you think??
I’m a 41 year old male looking to have circumcision revision surgery. (Since i’m about to get a Vasectomy, though i’d finally address this issue) The origial circumcision as an infant left me with skin that covers the corona and adhered to the head of my penis. There are 2 openings and another small pin size opening that i think happened because of a tear.  When the shaft of my penis is pulled downward it pulls the head down with it.  Can this be corrected and what who is the ‘best’ doctor in the Philadelphia, PA area you would recommend??  Thank you.    Benson H.

Good afternoon Benson,

Thank you for your interest in what we do.  Sorry, we do not maintain a directory of handful of urologists who are exceedingly gifted in cosmetic circumcision.

We do preputial bridges almost monthly here at the office.

There may be a slight pigment change at the base of each bridge pedicle, but usually not noticeable to a casual observer.   As a rule these areas are very thin, so this works to your advantage.

Yes, I am confident we can address your concerns and do an excellent cosmetic job.

I have performed over 6000 cosmetic circumcisions in the past 35 years and usually do several cases every week.  We have done sons of physicians, brothers, religious circumcisions, men who have not seen the heads of their penis for 20 years! This is your circumcision and we do it the way you want with respect to 4 parameters.

Please don’t be like the patient who writes: “Dr. Reed I just got circumcised two days ago and my result was not what i expected. I was wondering when i recover can i get a consultation with you.” Photos sent and received (unspoken comment “disaster zone”)

Be assured we will show you every courtesy,

All the best, and take good care of little Benson

Sure Benson , send photos.

The head of the penis is no stranger to malignant transformation and have such a patient now who had a partial penile amputation and a course of radiotherapy, going from 8 inches (he says) to about 3 measured at office.

If your bridges are covering desquamated skin, simple body dirt, smegma, you name it, best for that area to get a good washing on a daily basis.

More important than cosmesis (and we fuss over that) is hygiene.

Awaiting your photos,

Harold

Good afternoon Benson,

Thank you for taking time to send to photos.

What I am seeing is a corona (rim) that yearns to breath free.  I do not envision any problems with releasing what nature has given you and needs to be exteriorized.

Suggestion, let us do your vasectomy at the same.  Vasectomy fee is 500; if done as “an add-on” 400.

So far, how’s our driving?

Sincerely,

Thanks for your response.  I had recently sent photos to another Urologist who specializes in the same work you do.  From my research, there were only 2 doctors that I  found that appear to be experts and specialize in this type of procedure.  You and him.  He stated, my skin bridges were some of the most extensive he?d ever seen?.and it seems he?s seen a lot over the course of his career as well.  My skin bridges are consistent all the way around the head of my penis except for 2 small holes on the left and front side.  I?d say 98% of the corona is covered since the skin adhered over top the corona on the actual head.  So when the shaft of my penis is pulled down or towards my body?.it also pulls the head down too!

I?m very interested and am glad to hear that you do these monthly as you?ve stated.  I?m just a little ?wary? since the other professional thought that the head might be a little disfigured after the procedure and concluded that if I wasn?t presently in any discomfort or pain ?it?d be better to leave it alone.  Based on this information, do you still think you could make it right?

If you want, I could send you photos to give you an idea of what I?m talking about.  Please let me know.  I do thank you for your time and attention Doc.

Preputial (skin) bridge

Wednesday, January 20th, 2010

I sent you an email a little over a year ago asking about a skin bridge I have. After my in depth description of my bridge you replied that I had a preputial bridge.I would like to have this bridge removed for hygenical reasons as well as cosmetic, as its not very attractive. I guess my question is what would something like this cost approximately? I would like to have this procedure done in the very near future. I would appreciate any info you can give me. Thank you.     Daniel

Good afternoon Daniel,

Thanks for your continued interest in what we do.  Fees are as follows, consultation $250 and surgery for preputial bridge $500.  I assume you either have had, or are not interested in circumcision or circumcision revision.  Anticipate that the base of the bridge on either side usually 1 to 3 mm wide will heal with a small area whose skin color may be somewhat different from your present skin pigmentation.  Usually this is not especially noticeable.

Sincerely,

Harold M. Reed, M.D.
