Hi Dr Reed, I am 37 years old and I am interested in getting my phimotic condition corrected with circumcision. I understand that you use micro sutures used in plastic surgery. Are these self-dissolving sutures? Would I need to revisit you or go to a local urologist to have the sutures removed? Thank you for your interest in what we do.

We have performed over 6000 cosmetic circumcisions in the past 35 years and usually do several cases every week.  Many of our patients have phimosis so we well understand the principles of getting the best result.

Our sutrures while fast absorbing still should be removed 12 days after surgery to prevent permanent cross hatch marks (ghost lines and pin holes).  Please do not let anyone talk you out of having them removed, by saying “they are absorbable, they’ll melt out.”

This should be done by a health care practitioner who has a bright over head light, loupes (eyeglass with little telescopes on  the end) and delicate plastic surgery instruments  like for removing eyelid sutures)  EMLA cream could be applied for 20 minutes beforehand if you are very sensitive.  Some urologists have this equipment in their office if they do delicate surgery, or consider an urgent care center, or an emergency room of a quiet community hospital (not a county “zoo”). 

Next to the nose on your face, your penis is a very important cosmetic structure and standards for care should be no different.

We do ask that you stay for one, ideally 2 days at the Baltic Hotel (advanced tourist class) and I will see you there daily.

You could return to an office environment 3 days after circumcision.  Our 2 layered wrap of soft conforming gauze and Coban will provide comfort and reduce swelling.

We have done sons of physicians, medical students and residents, brothers, religious circumcisions, men who have not seen the heads of their penis for 20 years, and next week a father and 3 sons! 

This is your circumcision and we do it the way you want with respect to 4 parameters.

Please don’t be like the patient who writes: “Dr. Reed I just got circumcised two days ago and my result was not what i expected.   I was wondering when i recover can i get a consultation with you.”   Photos sent and received (unspoken comment “disaster zone”)

Unexpected or unexplained neonatal circumcision

Good morning Alicia,

Sounds like the circumcision of your son was not properly discussed with you.

Despite your misgivings now, a circumcision does confer many advantages health wise.  Such as avoidance of phimosis, decreased likelihood of urinary tract infections, deceased likelihood of his acquiring HIV and sexually transmitted disease.  It could be reversed later on after adolescence.  There are conservative (non-operative) means to do this, but chances are your son will appreciate being circumcised.  Doctors need to take time to talk to their patients and the family, so that you can make an informed decision.

Phimosis patient wants to know how to prepare for circumcsion

i cant retract my foreskin at all because my opening is so small,and i am planning on getting circumcised.will i need to do anything special prior to the operation and will that cause any complications or any difficulties during or after the procedure?

Hi Jason,

Thank you for your interest in what we do.  To answer your question, do not manipulate your foreskin or traumatize it for a few days before surgery.  Better for me to see what is actually phimotic (or contracrted) as that has to go.  Most probably you will not have a high circucmsion , could be moderate to low, but if I leave any scar tissue behind you’ll have big time swelling of ther distal shaft once that ring of scar tissue is re-sited above your head.

Overly tight foreskin, considering adult circumcision

Recent adult circumcision with puckers and other defects, what now?

Hello Doctor Reed,

It makes me relieved to have found your site and to know that there are circumcision specialists.

I underwent circumcision 7 weeks and I’m very dissatisfied with the results. Apparently the suturing was poorly performed, resulting in bumps that (to me at least) resemble warts. There’s also an area
around the scar line that folds inward (as seem in picture DSCN0474) when the penis is erect.

As for me it’s a long way to drop by for just a consultation (I live in Brazil), I was wondering if you could take a look at attached pictures and give me an idea of what I should expect from a revision surgery. Can I have a good looking scar line again?

The pictures were taken today, 48 days after the procedure.

Your photos have been reviewed and several thoughts come to mind. Until a doctor-patient relationship has been established, I do not wish to critique what you and I both see.

Fair to say you need to wait a good 3 to 4 months after your original procedure (adult circumcision) before a revision can be done. Your situation may improve somewhat without my intervention, but I am not sure, even then you will be happy unless a touch up is done.

Please initiate a phone consultation with our office. Puckers are often the result of widely placed, overly tight large caliber sutures, and regretfully may persist unless revised.

Japanese styled (ultra high) circumcision

Hello Barca,

The Japanese circumcision looks good on paper but if you have any significant tightening of the distal foreskin (phimotic ring), dragging that up as high as possible will constrict the base of the shaft and cause lots of distal edema which may only go away with revision surgery. If this is what you want, best to have the front side and back side done at 2 different times, or something still high, but less extreme. Also remember a dragged inner skin is a distal based flap, in other words getting its blood supply from the end of the penis, then taking a U-turn to go back to the pubis. The blood supply is therefore attenuated. This can result in unsightly scarring.

Another suggestion, and I am not trying to be cute, is to go to Japan to be under the care of a doctor who does this frequently with minimal complications.

Also please keep in mind the inner skin which you wish to preserve is the portal for many sexually transmitted diseases, even HIV.

Why leave the door open? Take good care of little and big Barca.

Closing a “dorsal slit”

I would like inquire about a dorsal slit reversal. Is it possible? if it is, will the outcome be just like an intact penis with all the foreskin restored?  Please give details regarding the costs of operation and recovery period.  Thanks in advance.   Matthew.

Good morning Matthew,

As you may know, a dorsal slit is a longitudinal release of the foreskin on the top side which opens the phimotic ring and usually results in a waddle of skin under the head and distal shaft.  Not that pretty, but often convenient when the patient may not be a suitable candidate for circumcision, such as  a gent with a severe penile infection.   A circumcision could be done secondarily.

Thank you for your interest in what we do.  Several thought come to mind.  Reversal is possible with some caveats.

A dorsal slit is usually done by a crushing clamp technique to reduce bleeding followed by an incision over the crushed area.   The edges are then oversewn to prevent delayed bleeding.  This reduces the amount of foreskin available to reconstruct unless you were very generously endowed to begin with.

Normal for men is a preputial ring at the end of the foreskin which upon reconstruction is the weak point and could lead to tearing with erections.  So while an attempt can be made to do this, there is no guarantee everything will be smooth sailing afterwards.

Fee for reconstruction 1500.    You will need to be on an anti-erection medication for 6 weeks.

Phimosis and preputioplasty (limited circumcision) requested

Please keep this in mind, if you have a phimosis, all unhealthy non-stretching tissue has to be removed or you will need a dorsal slit.   If you have scarred prepuce left, that will not retract over the rim of the glans for daily hygiene.  What you may wind up with could resemble a partial circumcision.  Also keep in mind the remaining prepuce normally swells quite a bit after surgery, and you will need a Q-tip for a few weeks to cleanse the inner recessed area.    But yes, it can be done.

Phimosis, expect lower circumcision

Had a very wonderful Greek patient here 2 days ago with retractable phimosis, but cracked and was painful after sex so much, that he needed a week to recover every time he penetrated.  Time for cosmetic circumcision.

Yes, by touching and looking at the white inner skin, I can tell exactly how much is inflamed and non-elastic.  This definitely has to be removed, otherwise the problem will recur.  With careful approximation, was able to do this without a lower V-cut.  He wanted the frenulum preserved, and that’s OK.  It was not hard, cord-like and fibrotic. 

Seen the next day at the office with his beautiful girlfriend and is doing well.  Not pain whatsoever.
