Unsightly pubic scar from phalloplasty, what to do

Dear Dr. Reed,

Had a combined penile procedure which resulted in some loss of length, and in my pubis, a divot is seen along with a spreading scar extending onto my scrotum.  Plus there’s a bald spot in my pubic hair line. 
Is this something you can correct?

Hi Ken,

We see these scars almost every other month at the office and the reason is distal based flaps usually do not heal well, unless many precautions are taken.  The reason is the blood supply which starts out in the heart is flows down (that  part is OK) but is then turned around and expected to go up or make a U-turn. 
In pre-operative anatomy, blood flow is normally not like that.

Combining length and girth, primrose path to disaster

Which procedure do you recommend for girth enhancement? Which one feels the most like pre op flesh?  Would girth enhancement alone prevent the penis from shrinking and do you gain any length from the girth procedure?  Do you use any implants in the lengthening procedure to prevent retraction?  

Can any type of girth procedure be done together with lengthening?
Would I be able to drive home 3 days after the procedure (10 hr drive)?   Calvin

Thank you for your interest in what we do.

Either AlloDerm or BellaDerm would be acceptable and in our hands produce similar results.  No sir, do no combine length and girth enhancement as the results for length are lackluster at best and often disastrous.  We have patients coming to our office every month who have had “get-there-itis” and rue the day.   You cannot drive home or go to the mall after the procedure.  Right to bed at a nearby motel and bed rest for 4 days.  Yes, sir. 

You want a beautiful result or do you want your penis to have more wrinkles than a stewed prune?

Girth enhancement will not prevent the penis from flaccid contracture as the material is soft (like a wet chamois).

We tried implants for length about 12 years or so ago, and desisted because of the overly high incidence of extrusion of the silicone strips.  We have also heard repeatedly that other physician(s) who employ this technique are (in the here and now) having patients with similar experiences.

Not all that glitters is gold.

Interested in BellaDerm

I am interested in the belloderm procedure. My measurements are 6 x 4.5 what sort of gains would I expect. do the grafts reabsorbe over time. I live in Australia, how would I have to stay in USA for recovery. And how long is recommended to have off work, I work in construction.

Thank you for your interest in what we do.

The grafts may, (may) reabsorb by about 10% but this is more than offset by replacement with cells of your own making. Please expect some added girth from the bruise effect which will subside, but afterwards the wind up is you should have a noticeable difference in girth.

Please avoid any possibility for genital trauma or straddle injury for a good 6 to 7 weeks. Yes, if caution is exercised you could return to work earlier, perhaps 2 weeks after surgery.

The California Medical Board has accused an Orange County plastic surgeon of sexual misconduct, including having sex with a patient in the examination room.

Dr. K  faces having his license revoked for two instances cited in the board?s accusation.

According to the documents, in 2009 Knight had sex with a patient while her husband and children waited in the parking lot. When the patient?s husband brought the children inside to use the bathroom, he heard moans from the examination room. There, he discovered his wife having sex with Dr. K.

K, 41, declined to comment Tuesday to the Orange County Register

That being said, pills and creams do not work (thus far).  Traction does for length only, when used under the guidance of a knowledgeable urologist.   I would propose you have your circumcision revised first, as traction which of necessity needs to applied after penile lengthening will rip your suture lines apart.  Also the penis does not appreciate too much simultaneous surgery.

Any verifiable news reports, corrections  or followups would be appreciated

Phalloplasty, weights and traction

Hello Doctor I was wondering if you could help me with this question.  After penis lengthening surgery when weights are required for 8 hours a day, how is this done?  Does that mean that the patient would stand with weights on his penis for 8 hours daily?

Good afternoon Bernie,

When we think of weights, more generally we are speaking of  traction which should be started a day after surgery simply with manual extension for 2 minutes (with you lying down) up towards the ceiling for two minutes and down (towards your feet) for two minutes, a least 8 times a day.  Give your penis a chance to accommodate.  You will notice towards the end of the two minute interval your penis is longer.  Also immediately following surgery, your penis has been wrapped, extended outwards, like in a cervical collar.  This helps too.

After 5 days the wrap is removed and slowly you will progress into serious traction graduating up to 8 pounds if your doctor feels your diameter is capable of that much tension without injury.

I could talk for many minutes about how traction should be and not be applied.

You can accomplish a good deal of the minimum 8 hours a day needed while on bed rest.

Sorry the information is vague, because without knowing you, or having a doctor-patient relationship with you, I cannot go into specifics, but I am available for consultation over the phone if you wish.

Our fee is 250 and this is a one time charge, thus there is no charge for your first visit at the office, excluding any lab work which may be needed.

BellaDerm vs. AlloDerm Phalloplasty for Girth

I am thinking about PE surgery and wanted to know the difference between alloderm and belladerm.  What are the differences between the two and which one gives a superior results as far as girth enhancement?

After girth enhancement surgery, with either alloderm or belladerm, can a second girth procedure be done?

How long after lengthening surgery can you start to stretch with thegrip?

Any thoughts on the Dr. E’s silicone implant and the late Dr. Perovic’s bio-scaffold?

How was it possible the patient in example#2 was able to get such a great girth enhancement?

Are you going to post some before and after pictures of patients with glans enhancements?

You are probably aware of the forum, mynewsize.com/forum and the owner/moderator, C. H, had trouble with his 2nd girth surgery. Good or bad, your name is not mentioned all that much on that forum.

Can the $250 consultation fee be applied towards surgery?

Thank you for your interest in what we do.

As consultation (by us) is an independent event, so we do not  co-mingle or reapply fees.  Nor do we try to sell.  We have told many a well hung man he can rest on his laurels and no surgery is needed.

Taking your thoughtful questions in  the order posed…

What are the differences between the two and which one gives a superior results as far as girth enhancement?Personally I do not see any difference in the ultimate result.  AlloDerm comes from a deceased donor, BellaDerm from a live donor (usually tummy tucks with massive amounts of skin removed).  The same corporation that sells BellaDerm also sell deceased donor products as well.

 

The maximal thicknesses are comparable, but BellaDerm comes in 20% lager widths, so when patients are buying volume and the doctor is charging the same amount for either, BellaDerm provides more volume for the same money.

After girth enhancement surgery, with either AlloDerm or BellaDerm, can a second girth procedure be done?Yes, but I think you’ll be happy enough with one application that you should not have any more girth enhancement surgery.

 

How long after lengthening surgery can you start to stretch with The Grip?We have our patients doing penis pulling exercises (manually) the day after surgery and it is not painful.  The Grip could be applied about 5 to 6 days after surgery (the zero day).

 

Any thoughts on the Dr. E’s silicone implant and the late Dr. Perovic’s bio-scaffold?Dr. E claims good experience with the product.  Our early experience with “crescentic” molds despite use of permanent mltiple Prolene sutures showed that as the spermatic cord occasionally lifts the testicle which is a normal thermo-regulatory mechanism, the capsule which tended to fixate to the scrotal wall separated from the testis and some patients reported “empty cup”.  We also did seminal analyses before and after cups and were really taken aback when they dropped to near infertile levels. 

 

Dr. Perovic my dear recently departed friend introduced the scaffold also pioneered by Dr. Kim from Korea.
At first they were impregnated with tissue culture cells of the implanted patient to permit more inward cell growth.

Dr. Atala noted research worker on tissue engineering and past president of the AUA has said if a scaffold is one cm or less in thickness, prior implantation of tissue culture cells growth is not needed.  Ingrowth of cells follows blood supply and capillaries will grow into a one cm. matrix without difficulty.

As for the results, I would suggest you speak to patients who have had this done.  I personally never used this product because we had excellent success with AlloDerm. 

How was it possible the patient in example #2 was able to get such a great girth enhancement?Example #2 had 3 layers of AlloDerm put in over 1 1/2 years.  We don’t recommend that, but this was his insistent wish.

Yes, when we have some mature results.

Penile silicone disaster patient seeks a better way

Hi Dr. Reed,

I found this email address on www.penisdoctor.com, I am hoping to arrange a consultation regarding a variety of things!

First of all, a few years ago I received silicone injections – 60cc in my scrotum and 10/20cc in my penis.I love the sensation of having a foreskin ( from the silicone in my penis shaft, I was circumcised as a kid), I love the size and the way it feels, however, I am dissatisfied with its appearance. My penis shaft is lumpy, there is a small area that looks inflamed, it looks inflated when flaccid rather than dangling down like a normal penis,

Therefore I am looking to reverse the silicone procedure, and would like to review other augmentation methods.

I am not sure if I am able to have everything done at once or whether it would have to be done in stages, so please provide me a breakdown of costs, and what you would do for the following:

1. removal of silicone
2. foreskin restoration (do you do this surgery?)
3 penile lengthening and augmentation
           (there seems to be lots of techniques in AlloDerm, scaffolding, fat transfer etc – what’s the difference in using each of these )
4. glanular enhancement
5. penis lengthening
            (is there a way to disguise/minimise the scars)
6. testicular enlargement

I have part-time work in adult entertainment, which I love to do. Therefore I want to go as long, and wide (within sensible limitations) as possible.

BellaDerm from Musculoskeletal Foundation MTF
