Good morning Adalberto,

The length result is more dependent on dedicated use of traction than surgery or after surgery, although surgery can be a facilitator.  Surgery typically provides only 3/8? increase in length if you use traction as directed for 3 to 4 weeks afterwards.

While every doctor has some very successful cases, truly the patient gets 65% of the credit.  Assuming your weight remains stable, whatever you gain is permanently yours.

Good morning Adalberto,

Thank you for your continued interest in what we do.

Doing length and girth at the same time is not advisable for so many reasons.

Just to name 2, first applying traction the next day after lengthening by doing the pulling out and down exercises would not be possible (too much pain and possible rupture of the incision line), second if your penis is going to be longer you will need appropriately longer strips to enhance girth.   If you have lengthening only you can do these exercises a day later and apply traction in about 5 days without any significant discomfort.  What a difference.

But I’ll mention 2 more reasons not to do both at the same time.  the penis does not appreciate too much surgery (think impaired healing) and next…  girth enhancement products such as AlloDerm, BellaDerm will only stretch so far with traction.  They are a dermal matrix essentually devoid of cells.  Your native penis is different.  That elongates by virtue of new cell growth in a longitudinal direction.  Stretching is only the inducement.

I am looking for a surgeon who does both fat transfer and alloderm. This way they know the difference first hand and know the short/long term results. I’d like these questions answered please: Which procedure do you suggest? Do you use lasers? Is their erect length gained? Does the penis stay at erect length and not grow with fat transfer? Does the penis feel real and not too soft with fat transfer? What is the average of erect girth gained for each procedure? (I’ve seen 30% flacid and 5-10% erect) These are the questions I have. You are the only surgeon I have seen that does both procedures. I’d like to come to FL to have this done, assuming you are confident in positive results. I’m 3′ flacid length, 6.25′ erect length, 4.75′ flacid girth and 5′ erect girth. Please let me know your thoughts. I appreciate your time!  Junior

Yes generally, but I do not recommend fat transfer because of many potential complications.

What girth method is permanent?

my friend went to dr so and so and after surery his penis was huge and he was happy after it the swelling went away he didnt see much differece. If i choos eyou and I have this problem would i be able to go back to you for free of no charge to correct iit?

Good afternoon Jason,

We have not seen AlloDerm girth enhancement dissipate once the final result is reached, that is after the bruise effect subsides.

I am of the opinion if you have not have previous girth enhancement procedures, from one “extra thick” AlloDerm you can expect about 3/4″ increase in circumference.  If you do not get this, yes I will insert an additional strip about 8 months later to permit the first strip to have sufficient capillary ingrowth.  Putting one complete strip on top of another is not a great idea in that there is a greater chance bacteria may seek refuge between the layers.

Penile enlargement questions

I have a few questions for you. If you could answer them  with some good information for me.

1. My girth is 4 1/2 erect. That’s quite skinny. Can i get to a girth of 6 inches through your surgery ?

2. The alloderm once it’s in the penis…when it’s erect, how does the alloderm feel ? Is the penis still rock hard like my erections now ? Or is it kinda squishy and you loose a little of the firmness and hardness ?

3. Does the surgery affect the erectile angle of the penis ? Angle is actually extremely important in sex and having a good angle around 2 o clock position is a huge plus. Would the surgery affect the erections angle so it points lower and cant rise to where it once was ?

4. Do you have any erect before and after pictures ? I see ones on all the sites in flaccid but im interested in the erection as well.

5. Is there any way to determine how much penis is inside a man’s body for lengthening surgery ? What is the average amount inside and what is the largest amount you’ve seen ?

Seems to feel by and large like normal penis. If your penis flaccid is not soft and squishing now, neither will
it be with AlloDerm, nor will it be that way erect.

Peyronies has produced significant shortening and ED

The best results for Peyronies’s without question come when there is simultaneous implant of an inflatable prosthesis, which should not be done if there is sufficient wherewithall to penetrate.

We are seeing several Peyronie’s patients every month.  One memorable patient from Ohio was done with a length gain of 1 1/4″ immediately on the table.  When the patient arrived in the recovery room and said to me, “how did I do,” I drew the curtains around his bed and pulled back his sheets and blanket and said, “look.”  An approving smile came across his face.

With post operative use of a vacuum erection device, he’ll even do better than that over the course of the next 6 months.

Correcting penile curvature is one of the mainstays of our practice and the enjoyment we derive is making two people happy.  We see patients from all over the world who seek remedy because of functional or cosmetic disabilities.

My knowledge is updated yearly by attending the AUA meetings.  At any lecture dealing with Peyronie’s you’ll find me sitting very attentively in the audience. 

While our surgical success rate is better than 95%, we do recommend a trial of conservative therapy for at least 6 months as this may often produce a sufficient enough resolution to make you a happy man.  But have you had this without success?

If you are over 40 and are on medications other than hormones, please call the office before scheduling surgery,  as a letter of medical clearance will most likely be needed.

If you are a smoker, you must stop completely for 2 months before surgery and please never again.  Smoking causes vaso-spasm, impairs wound healing, produces excessive scarring, tissue necrosis,
irritates and inflames pulmonary membranes and enhances anesthetic risk.

This goes also for any products containing nicotine or nicotine like agents including nicorette gum, nicotine patches, Nicotrol inhalers, and second hand smoke.  Again not for 8 weeks before and not for 8 weeks afterwards.
Hopefully never again.

No aspirin or aspirin products, blood thinners such as Plavix, Coumadin, no anti-inflammatory drugs such as Motrin, Advil, Alleve, or Cox 2 inhibitors such as Celebrex or Vioxx for a week before surgery.  No Vitamin E or excessive alcohol or spices for a week before surgery as this may promote bruising.  Bruising can be reduced (many believe) with Bromelain 500 mg 1 twice a day, Arnica montana dose 30C (take 4 small tablets sublingually, 4 times a day), vitamin C 500 mg (3 times a day) starting 1 week before surgery.

Thank you for your interest in what we do.

The grafts may, (may) reabsorb by about 10% but this is more than offset by replacement with cells of your own making. Please expect some added girth from the bruise effect which will subside, but afterwards the wind up is you should have a noticeable difference in girth.

Please avoid any possibility for genital trauma or straddle injury for a good 6 to 7 weeks. Yes, if caution is exercised you could return to work earlier, perhaps 2 weeks after surgery.

Are there any specials right now?

No.

Where are the incisions made for this procedure?
Under the head of the penis? At the base?

Yes there is a 9 to 3 o’clock incision in the coronal sulcus,
the groove in back of the head of the penis and a vertical incision
about 1/2 to 3/4 on an inch long at the peno-pubic junction.  Both incisions
are closed very cosmetically and usually heal quite neatly
and fairly inconspicuously.

What is the recovery time for this procedure?

You need to be on strict bed rest for 4 days at the Baltic Hotel
following procedure, and keep a wrap on for 7 to 8 days.

How long before intercourse?

About 8 weeks,.

I read patients who undergo this procedure must stay 4 days, is this still standard procedure?

Absolutely.  A.  To prevent shifting and sliding of the dermal-matrix product,
