<p>Both are ways to correct a penile-scrotal web.<br />

The Z-plasty will leave a Z scar and the V-Y plasty  conceals better for the reasons I have mentioned in earlier letters. I am also of the opinion the V-Y will make your penis look longer because the skin wraps around the<br />
base better.<br />

<p>Yes, we use the Y-V plasty for many reasons including the fact the  scrotal lines or rugae are shaped like a Chevron and the incision lines tend to fall in the creases and  are more concealable.</p>
<p> There may be a small scrotal dog ear on either side to clip off  truly representing less than 1.5%  and usually less than 1% of the total scrotal surface area, more than  offset by the added  scrotum from your penis that is now a part of the true scrotum.</p>

<p> We have done many peno-scrotal webs and could do a Z-plasty.  but those I have seen from other offices, look like the mark of &#8220;Zorro&#8221;  although the problem is corrected. Because you have a midline pigmented structure called the raphe, the incision line is begging to be put there (IMHO).</p>

In searching for reviews about your practice, I discovered a couple of negative reviews that have me concerned.  While I understand that the veracity of one person&#8217;s vitriolic review can never be adequately understood, nor the highly subjective nature of this person&#8217;s rant assessed, I am concerned about one criticism in particular.  The critic wrote that s/he was misled regarding costs of the surgery.  While s/he was quoted the same costs as those you informed me about ($250 consult; $2500 for surgery, anesthesia and facility fees), the writer maintains the costs quoted at the consult were closer to $6000 for the surgery.  Please respond.  <br />

<p>Yes, my office is in greater Miami.</p>
<p>The CDA 230 (section 230 of the Communication Decency Act)  permits<br />
people to write negative reviews about any business without any concern for libel, and they may often re-write another item under another assumed name or  E-mail address to lend more import. </p>
<p>HIPAA is a federal law which ensures patient confidentiality, so physicians cannot respond in kind without violating HIPAA.</p>

<p>Fees with patients are established before surgery, and on complicated cases best determined when they visit our office for the first time and are examined.  When patients request ad-ons, the initial estimate does not apply.  For conventional surgery such as a circumcision, a penile implant, insertion of testicular implants, the fee is standardized. </p>

<p>Think of the scrotum as a hammock on which the testes will rest after orchidopexy.</p>
<p>If the scrotum rises, the testes will also, but should always remain on the bottom of the scrotal compartment.</p>
<p>We are contracted Blue Cross, Blue Shield providers.  Please call Anne to validate your plan. </p>
<p>Peno-scrotal web as a procedure unto itself is 2500, and orchidopexy when done simultaneously is 4500.</p>
<p>Pubic liposuction should not be done at the same time, and does require a general anesthetic.  Our fee for that varies depending upon what has to be done, from 3500 up.</p>

<p>Thank you for your interest in peno scrotal web release, a procedure which I have been doing frequently for a good 25 years.</p>

<p></a>There is no question that if your erectile bodies extend well beyond the body surface line, having them wrapped solely with skin as opposed to a draping scrotum underneath will from many views provide the semblance of longer length, actually the true length of your penis.  When the penis is erect it tends to be elevated and when you are vertical the underside of your penis is what your partner sees.  This should be a turn-on.</p>
<p>Sexually the advantage is you will not be dragging your scrotum into the vagina during penetration.</p>
<p>Will be leaving this Friday for the annual meeting of the American Urological Association, this year in San Francisco.  Have attended 37 consecutive annual meetings.  Plan to be back at the office, June 3rd.</p>

<p>Thank you for your interesting question. First let&#8217;s talk about overly tight circumcisions. Two &#8220;no-no&#8217;s&#8221;. You don&#8217;t want it that tight you see lifting of the peno-pubic angle or raising of the pubic skin to meet the needs of the penis, nor do you want your scrotum drawn into the base of your penis creating a peno-scrotal web.</p>
<p>Next for those patients who request tight circumcision, the use of steri strips, left on for about 12 days should suffice. Also plan on being on bed rest for 2 days, after circumcision, in that wound separation is seldom caused by an erection alone, but more often by a simultaneous collection of fluid under the skin which impedes good wound healing. We mean being in bed within 8 to 10 minutes after the patient leaves our office. If the patient lives beyond that distance, he has to stay at a nearby motel. No shopping at the Mall or weddings for the next few days.</p>
<p>Lasting we do procedures such as foreskin reconstruction and after the first stage, have to bury the penis under the scrotum and of course, erections are to be avoided. For that spironolactone 100 mg twice a day (avoid high potassium foods) is what we recommend. This is an anti-testosterone drug. </p>
<p>To control nocturnal erections, the use of the old styled Sudafed (pseudoephedrine) 2, 30 mg tablets every 6 hours is helpful. Please read the brochure, can cause elevated blood pressure, etc.</p>
<p>Incidentally in our operating room, the patient gets a hand-held mirror and we make the circ is as tight as the patient wants (of course avoiding extremes, that will come back to bite us)</p>
