Transsexual male seeks penile implant

I am a 29 yrs old post op FTM. I had my pallopalsty done in 04 in Montreal with Dr. Brassard. Have been on hormones for 10 years.
Specific_Questions: I just wanted information on penile impants. I got testicular implants in 08 and the penile implant is the only thing to left to complete me. How much? Recovery time, etc…

Thank you for your interest in what we do.  I am glad you are 29 years old.  There will be time for a lot of activities.

Hopefully your penis is wide enough to permit 2 cylinders as we would do for genetic men.  You have a choice between inflatable and semi-rigid or malleable implants.

There are pros and cons to each.

Simultaneous urinary burning and inoperable penile implant

Doctor Reed, I have a penile implant that is about 12 years old. As of 4 weeks ago, I found that it is not working and now have burning while urinating.   My question now is: should I be in a rush to have a new implant or would it be better to wait for a later date? Also, how long would it be necessary for me to stay at your hospital?

Thank you for your interesting case presentation.

I am more concerned about burning with urination called dysuria, than an operable implant as the 2 do not necessarily  go hand in hand.

Dysuria usually but not always indicates a urinary tract infection, and this needs to be tended to first before you consider replacement of your implant.  Rarely dysuria can come about when an implant erodes through the urethra.

At any rate we would be pleased to look at you and have a 2000 case experience with implants.  The procedure for most insured men is done in a hospital setting and we do keep patients overnight.  I would then ask you to stay at a nearby motel for 3 days of so for followup, before you return home.

Combining length and girth, primrose path to disaster

Which procedure do you recommend for girth enhancement? Which one feels the most like pre op flesh?  Would girth enhancement alone prevent the penis from shrinking and do you gain any length from the girth procedure?  Do you use any implants in the lengthening procedure to prevent retraction?  

Can any type of girth procedure be done together with lengthening?
Would I be able to drive home 3 days after the procedure (10 hr drive)?   Calvin

Thank you for your interest in what we do.

Either AlloDerm or BellaDerm would be acceptable and in our hands produce similar results.  No sir, do no combine length and girth enhancement as the results for length are lackluster at best and often disastrous.  We have patients coming to our office every month who have had “get-there-itis” and rue the day.   You cannot drive home or go to the mall after the procedure.  Right to bed at a nearby motel and bed rest for 4 days.  Yes, sir. 

You want a beautiful result or do you want your penis to have more wrinkles than a stewed prune?

Girth enhancement will not prevent the penis from flaccid contracture as the material is soft (like a wet chamois).

We tried implants for length about 12 years or so ago, and desisted because of the overly high incidence of extrusion of the silicone strips.  We have also heard repeatedly that other physician(s) who employ this technique are (in the here and now) having patients with similar experiences.

Not all that glitters is gold.

Interested in BellaDerm

I am interested in the belloderm procedure. My measurements are 6 x 4.5 what sort of gains would I expect. do the grafts reabsorbe over time. I live in Australia, how would I have to stay in USA for recovery. And how long is recommended to have off work, I work in construction.

Thank you for your interest in what we do.

The grafts may, (may) reabsorb by about 10% but this is more than offset by replacement with cells of your own making. Please expect some added girth from the bruise effect which will subside, but afterwards the wind up is you should have a noticeable difference in girth.

Please avoid any possibility for genital trauma or straddle injury for a good 6 to 7 weeks. Yes, if caution is exercised you could return to work earlier, perhaps 2 weeks after surgery.

scar tissue from prior penile implant, can it be fixed

Thank you for your interest in what we do.  Sometimes, a less than optimal injection technique can be modified to  produce a good result, or the result can be augmented with a constriction band.  Also we should consider a vacuum erection device.  Please call the office and make an appointment.  These choices can be reviewed  and shown in detail

Patient satisfaction for a penile implant runs about 90% as does partner satisfaction (national averages).   Not too shabby.    A newer technique permits a somewhat longer erection than otherwise, if you should choose an implant.

Titan Pump – Coloplast

The FDA approved the improvements to the pump component of the Coloplast Titan prosthesis in July 2008. Although the cylinders and reservoir remain the same as the previous model, the pump now features a one-touch-release (OTR) deflation valve — easy for patients to locate and operable with one hand. In addition, the pump offers a non-bulky, low-profile size; enhanced silicone for higher threshold “tear strength” (likely to result in increased product durability, an issue with previous versions); and an overall simplicity likely to decrease repeat office visits, phone calls, and repeat training time. From a hospital standpoint, intraoperative prep of the device provides for easier priming of the implant system (the removal of excess air prior to filling) and may reduce slightly the overall intraoperative time.

“This remains a great device for the patient with a larger penis, as these are the only cylinders that will expand to 22mm girth.” In addition, only Titan cylinders are available in lengths of 24cm, 26cm and 28cm.

Although the pump is easily identified and operated by patients, the small size of the deflation valve sometimes requires a longer learning curve.

In a recent study (“Evaluation of Three Penile Prosthesis Pump Designs in a Blinded Survey of Practitioners,” Urologic Nursing, 2008), 32 medical professionals, all familiar in teaching the operation of penile implants to patients, reviewed currently available penile pumps. The blindfolded reviewers, examining the pumps under time constraints through mock scrotal sacs, were asked to rate device:

 

--  ease of location of deflation valve
--  ease of inflation
--  ease of deflation, and
--  anticipated ability to train patients in clinical setting.

 

The Titan OTR pump design performed very well and “consistently demonstrated a significant advantage in subject preference.” (Quallich, Ohl & Dunn, 2008, p. 5).
