Fordyce spots

May 3, 2011

Hi Dr. Reed,

We spoke in August of last year regarding the tract marks that were left from my circumcision. Please read our emails below for a quick refresher.

I took your advice and went to see a dermatologist and explained the situation in its  full entirety. He gave me a cream called Dermovate  which is a steroid cream that I have to apply nightly. I don’t think this will help as it will not tackle the fact that the skin healed around the stitches creating tiny holes that seem to contain sebum sometimes. It seems to me that the only other way would be to excise the skin from the area.

How much would this procedure cost and how many days would I need to be in Miami for?

Also I have tiny Fordyce Spots on the shaft of my penis and would like them removed. I know they are not of any concern but for me I hate them. I have researched this tremendously and it seems the only method to tackle Fordyce Spots with a hyfrecator.   Do you do hyfrecatio treatments and if not can you recommend anyone to Hyfrecator these tiny Fordyce Spots.

These issues are affecting my sex life and as a young male who loves women, I really want them to get sorted.

Hope you guys are doing well in sunny Miami,

Kind Regards,
Dillon

Good morning Dillon,

Thank you for your update. Most likely we can do both if the Fordyce spots are not too extensive or clustered so that hyfrecation will leave a noticeable scar on the skin. As you know Fordyce spots are small sebaceous cysts unassociated with hair follicles.

Out fee for both is 1800 to include office consultation, local anesthesia, use of the operating room, and my followup care.

Plan on staying either at Daddy O’s Hotel or the Baltic as I will see you there daily for 2 post-operative days.

Please review the attached for general suggestions.

Sincerely,

Harold M. Reed, M.D.

Do you induce an erection during surgery to be sure, it will not be too tight

Sunday, April 24th, 2011

April 23, 2011

I am terribly sorry for the delay of scheduling my appointment. I was caught up with some personal matters. In either case, I have been doing a little more research on peoples adult circumcision experiences and I have a very specific question regarding style and procedure.

Do you operate on the penis while it is erect to ensure the look is aesthetically pleasing with a full erection, or do you operate on it while it is flaccid? Is it bad practice to operate on a penis while it is erect?

My concern is that my penis is very small when flaccid (1.5 inches) but about 5.5-6inches when erect. The other cosmetic urologist I spoke to said he would not induce an erection neither before or during the procedure (indicating it was unacceptable practice) and he will estimate how much skin to remove only while flaccid with some manual traction to simulate an erection. This makes me worry too much skin will be removed without knowing how much the penis expands on an erection and as a result will cause my penis to become shorter in length.

What is your approach to this? Is operating on an erect penis truly more risky than while it is flaccid? How would we ensure that only enough skin is removed to maintain a good look when fully erect?

Thanks,

Pattash

Good morning Pattash,

While we do not operate on the penis when erect, we do put the penis on traction to simulate erect length.

Inducing an erection with medication leads to enhanced blood flow and may cause more bruising and blood loss.  Of course we could inject your penis with sterile saline and place a tourniquet at the base as we do for Peyronies’s patients, but this is normally not done.

If you wish, we could induce an erection in the examination room and pre-mark your penis. Then we could reverse the erection in the operating room.  So there are many ways we can accommodate you.

More importantly is that you like the result when your penis is flaccid.  For every patient we see that is unhappy with their penis erect post circumcision by another doctor, we see 20 times as many patients unhappy with the appearance when flaccid.  For example,  loose skin rolling over the corona or rim of the glans.

That too much skin will be removed is a good hypothetical consideration, but in fact has never happened in our practice.

Have a restful weekend,

I believe I need a frenuloplasty procedure. I am uncircumcised & during intercourse experience almost an extreme pain due to a reoccurring tear at the frenulum of my penis. Do you perform this procedure? Also, would the procedure be covered by insurance? If not, how much is the procedure? Please help & thank you!   Jorge

Good afternoon Jorge,

Called and left a message.

We can do frenuloplasty, or frenuloplasty with circumcision.

To best advise you, I would have to see you at the office which is in Bay Harbor Islands, next to the Bal Harbour Shoppes.

Please call Anne, our amiable office manager for scheduling.

Blue Cross/Blue Shield paying for circumcision

Thursday, April 21st, 2011

April 17, 2011

I would like to know what would be considered medical reasons to have adult circumcision, what are the criteria for it to be considered medical rather than cosmetic?
Does any insurance pay part of this procedure, I have Blue Cross Blue Shield.

I can be reached by phone any time during the week, thank you!

Best,

Pendor

Good afternoon Pendor,

Thank you for your interest in what we do.  While the good news is that most insurance companies do pay for circumcision and are fairly liberal,
although they would all insist on a medically valid reason, the bad news is they allow about 200 to 300 for the procedure, and this is why
some doctors tend not to exercise the degree of attention we do.

We have performed over 6000 cosmetic circumcisions and revisions in the past 35 years and usually do several cases every week.

We have done sons of physicians, brothers, religious circumcisions, men who have not seen the heads of their penis for 20 years! This
is your circumcision and we do it the way you want with respect to 4 parameters.

Please don’t be like the patient who writes: “Dr. Reed I just got circumcised (elsewhere) two days ago, and my result was not what i expected. I was wondering when i recover can i get a consultation with you.” Photos sent and received (unspoken comment: “disaster zone”)

Be assured we will show you every courtesy,

All the best,

Dire consequences of having prominent veins in one’s foreskin?

Thursday, April 21st, 2011

April 17, 2011

Hello Dr.Reed I’m 47 yrs old, I have two veins connected running through my foreskin and I’m worried about infection? Thank you for your time
Matthew

Good afternoon Matthew,

Thank you for your interest in what we do.  Having veins in your foreskin should not in of themselves cause an infection.  You may wish to have them removed, only if they are unsightly or the presence of your foreskin is
causing some special problem..  This is your call.

We have performed over 6000 cosmetic circumcisions and revisions in the past 35 years and usually do several cases every week.

We have done sons of physicians, brothers, religious circumcisions, men who have not seen the heads of their penis for 20 years! This is your circumcision and we do it the way you want with respect

Risky 57 year old seeks circumcision and doctors are rubbing their chin

Saturday, April 2nd, 2011

(editorial comment: proscribe means prohibit, condemn, denouce or forbid)

Thank you for providing a forum for discussion on this issue. I am 57 year old male int the Chattanooga, TN area, who has recently been diagnosed, by my GP, with phimosis and he proscribes circumcision as remedy, comically referring to it as my, “sex change operation”. I’m still looking for the humor. He has arranged an appointment with a urologist in Chattanooga but I am uncertain how best to assure myself that I have received the best opinions before I make my final decision.  I’m not sure what specific help you might offer but any referrals to information or anything that you might offer as general guidelines would be most appreciated to say the very least.

I have only lived in this area approximately 3 years so have no local network nor history to draw from and frankly I have been frightened by such frivolous attitudes as that described above.  Since being in this area I have been prescribed anti-inflammatory meds which specifically state “not to be prescribed for persons with heart conditions”, I have had two heart attacks and house six stents within my chest.  This prescription came from the same GP referenced above.

Specific to the circumcision issue I understand that some bleeding is to be expected and can be excessive.  I take Plavix daily as well as low dose aspirin and have been refused orthoscopic surgery to deal with a simple torn miniscus in my left keen because of this.  How much additional risk does this present in circumcision?

I am floundering right now Doctor and would just like to try to assure myself that I fully understand my options and possible repercussions.  I have undergone three previous out-patient surgeries which were described as, “common”, “standard”, even, “a cake walk”.  All three have left me with life-altering after effects so if I sound a bit neurotic or paranoid, maybe I am.

With sincerest thanks,

Mac

15 year old seeks circumcision

Saturday, April 2nd, 2011

April 1, 2011

Hi, I am 15 and am looking for a circumcision that would look similar to that of a boy who was circumcised after birth.  Basically I prefer not to have a bad scar. And a normal look.  I have also had sex, will this affect my circumcision?
Thanks,  Amos

Good afternoon Amos,

Thanks for visiting our web site

In that my license does not permit me to counsel or treat minors routinely without parental consent, I would welcome a conference call with you and your parents on the line.  I’m sure we’ll have a lot to talk about.

Best wishes,

Legally blind man wants cosmetic circumcision

Saturday, April 2nd, 2011

March 31, 2011

Thank you for your speedy response.  You said Most insurance companies pay a doctor about 300 for circumcision.  I can handle 300 with no problem.  Is that the max that it would cost for the surgery alone?
You listed some reasons for having it done.  Surprisingly you mentioned a problem that I have, uncomfortable sex.

I was married for 25 years and sex was not enjoyable. My wife had passed away 6 years ago. If I re-marry, I don’t want to go through the same experience again.  Why other guys have no problem with their sex lives being uncircumcised is beyond me.  I did.  Go figure.

You didn’t reply to my transportation question. If transportation is not readily available and near by  I  can’t come.  I am legally blind and can’t drive. How far is the hotel from your clinic where I would spend the night?

Also you did not reply to my question for after the surgery, do I have to follow up with my doctor?  Yes that one doctor did laugh at me that’s why I don’t want to tell my present doctor.  I called the urologist some time ago here in Saginaw and asked if they did adult circumcision.  She said yes.  After answering my other questions I heard the receptionist laugh as she hung up.

Was she laughing at me or of something else I don’t know but due to my past experience I took it personal. Oh by the way I am 58 years old.
And one last question for now that you didn’t respond too, how much discomfort is there after the medicine wears off?

Thanks again,

Ernie

Good afternoon Ernie,

Our fee is 250 for the consultation and 1750 for circumcision .

That alone is 2000 and transportation and hotel room for 3 nights could
be another 700 more.  Is this affordable for you.

If not, then please have you circumcision done locally.  Hate to say this
but if you are legally blind, you may not be as fussy if you have a
standard circumcision.

Mean no offense but let’s be practical and economical as well,

Harold M. Reed, M.D.

Getting Elton off to a good start with many questions about circumcision

Saturday, April 2nd, 2011

March 31, 2011

Hello Dr. Reed, Thank you for your phone response. I have several questions to ask you. You had mentioned on the phone that this circumcision is considered cosmetic surgery. Will my insurance cover that? If not what would be the total cost to me. Do you require cash or can I make payments? You had mentioned different types or styles of circumcisions, is there a difference in price depending on what type/style I choose. I would want the full circumcision if there is such a thing. You had mentioned that you don?t have an office here in Mich but suggested that I contact a urologist here in my area. I have to be referred to them by my doctor. I was laughed at once before by another doctor and now I am embarrassed to approach my new doctor. Also I don?t have a good reason except that it is a preference that I have and my doctor will probably tell me (as did the first doctor) that the insurance won?t cover it. What kind of a reason can I give for having it done? Do any of your patience have this
contact: Email
how: Just stumbled across it. Elton

Good morning Elton,

Taking your thoughtful questions in the order posed…

You had mentioned on the phone that this circumcision is considered cosmetic surgery.

Whether it is done for medically valid reason which include but not limited to recurrent tears, hygienic problems with inflammation, tightening with limited ability to retract, bleeding from sexual trauma, uncomfortable sex, etc, we will perform it as cosmetically a we can.

Most insurance companies are reasonably liberal with indications.  Typically they will require that the specimen be sent off to pathology for examination.  We cannot tell a lie nor should you, as that would be medical fraud.

Please confer with a representative of your carrier and pose the question directly based upon your indications.

Will my insurance cover that? If not what would be the total cost to me. Do you require cash or can I make payments? You had mentioned different types or styles of circumcisions, is there a difference in price depending on what type/style I choose.

Please confer with your insurance company for “out of network” provisions.  That refers to when a doctor is not contracted or does not agree to accept your insurance, the insurance company will make out a check to you directly.  Most insurance companies pay a doctor about 300 for circumcision and that’s why some doctors don’t spend as much time as we do.   If you wish us, you do have to be prepaid by surgery and usually make a 50% non-refundable deposit to schedule.

I would want the full circumcision if there is such a thing.

Of course.

Laughing at you.  Really?

Hope that gets you off to a good start.

Saturday, April 2nd, 2011

March 30, 2011

Specific_Questions: I have an 18 yr/old nephew that would like to get circumcised. I had contacted you in the past for information & would like to request it again. He is a senior in high school & has been on the swim team for 3 yrs. He was not able to consider the surgery during swim season & he would like to move forward with it at this time. Kindly forward the necessary information so that he can become more informed as to the type of cuts/procedures available as well as possibly scheduling a consultation. I look forward to hearing from you. Sincerely, Jeanne

Thank you for your continued interest in what we do.

Please show this to our nephew.
