Unhappy camper needs revision of earlier circumcision

Hi Dr. Reed,

I am interested in circumcision revision. I was circumcised at age 11, I’m now 32. Since I was old enough to know the difference, I’ve been very unhappy with how my penis looks. Based on my research, the procedure was done freehand using sutures. The incision line is uneven and I have a couple dozen suture scars around the top of the shaft just under the glans. (Kind of like railroad tracks.) Most of the suture scars are quite thick and pronounced. Many of them touch and even enter the corona. My question is: could a revision help clean up the look of my penis? I know it will never be “perfect” but I would love to lose the ragged, scarred look if possible.   Matthew

Very loose previous circumcision, smarter now

Dr. Reed, I would like your services for circumcision. Due to travel, it would be wonderful if this could be done on June 4 or June 7,8, or 9. I am 33 years old and have been previously circumcised, but very loosely so that I appear to be completely uncircumcised except for a scar far back on my penis. I would like this scar line to be part of the skin removed in the new cutting to avoid having two scars. I would like the frenulum to be totally excavated and the result to remove as much of the inner skin as possible. I want the glans to be totally exposed at all times with no skin bunching behind, but I also want to have a full erection with no discomfort.  Lenny

Had circ, skin bunches on underside

Hi Dr. Reed -

I am interested in the possibility of having some excess skin removed that bunches on the underside of the penis. This wouldn’t be a circumcision, as I am already circumcised, but more of a ‘penis lift’, I guess, for lack of a better term.  I’m concerned about preserving a particular area of sensitivity on the underside, however, so I’m just curious about what my options, if any, might be for something like this. 

My guess is that a consultation may be in order.  I am in New York, however, and I’m wondering how you go about scheduling consultations for out-of-towners.  Do you request photos first to get a sense of things? If you are able to determine if things can move forward, are consultations and procedures usually scheduled within close proximity, or are subsequent visits are usually made? In this case, though, I suppose it’s best to see first what can be done with my particular situation.

Thank you for your interest in what we do.  If you will, E-mail me several well focused photos of yourself.  Bunching of skin on the underside may relate to the fact the doctor did not do any even approximation all the way around, which could have been accomplished (usually) if he bisected his suture approach (6 o’clock followed by 12 o’clock followed by 3 and then 9 and so on.   However, we can correct this now.  You may have a longitudinal incsion on the underside, but this should heal fairly neatly.

Phimosis patient wants to know how to prepare for circumcsion

i cant retract my foreskin at all because my opening is so small,and i am planning on getting circumcised.will i need to do anything special prior to the operation and will that cause any complications or any difficulties during or after the procedure?

Hi Jason,

Thank you for your interest in what we do.  To answer your question, do not manipulate your foreskin or traumatize it for a few days before surgery.  Better for me to see what is actually phimotic (or contracrted) as that has to go.  Most probably you will not have a high circucmsion , could be moderate to low, but if I leave any scar tissue behind you’ll have big time swelling of ther distal shaft once that ring of scar tissue is re-sited above your head.

Self-conscious about scarring and railroad tracks after circumcision

To whom it may concern,
I was circumcised as an adult approximately 3 years ago at age 39  following some complication as a result of phimosis.  Overall the surgery went very well with no complications and very little discomfort, however, I am finding myself wondering whether I have done my homework and chosen the right doctor for my procedure.

While I am perfectly happy being circumcised and I have no regrets having my foreskin removed, I am finding myself somewhat unhappy and self-conscious about the scaring, those ?railroad tracks? left as a result of the freehand circumcision.

After visiting your website I wanted to reach out to you to see about my options if in this case. Given that you have done a number of corrective procedures and revisions, I have hoping that you might be able to offer some advise or help with my situation.

Thank you for your interest in what we do. While we only accept Medicare and most types of Blue Cross, Blue Shield, we will be happy to fill out your insurance form, for maximal reimbursement to you.

Virtually 99% of our patients are done under local and feel no pain whatsoever. Recent adult circumcision with puckers and other defects, what now?

Hello Doctor Reed,

It makes me relieved to have found your site and to know that there are circumcision specialists.

I underwent circumcision 7 weeks and I’m very dissatisfied with the results. Apparently the suturing was poorly performed, resulting in bumps that (to me at least) resemble warts. There’s also an area
around the scar line that folds inward (as seem in picture DSCN0474) when the penis is erect.

As for me it’s a long way to drop by for just a consultation (I live in Brazil), I was wondering if you could take a look at attached pictures and give me an idea of what I should expect from a revision surgery. Can I have a good looking scar line again?

The pictures were taken today, 48 days after the procedure.

Your photos have been reviewed and several thoughts come to mind. Until a doctor-patient relationship has been established, I do not wish to critique what you and I both see.

Fair to say you need to wait a good 3 to 4 months after your original procedure (adult circumcision) before a revision can be done. Your situation may improve somewhat without my intervention, but I am not sure, even then you will be happy unless a touch up is done.

Please initiate a phone consultation with our office. Puckers are often the result of widely placed, overly tight large caliber sutures, and regretfully may persist unless revised.

Thank you for your interest in what we do.  Using a block is routine for us and that is included in our fee.  We do recommend that you convalesce (bed rest) for 2 days at the Baltic Hotel before departure.  Typically you could return to a office environment 3 days after your circumcision.  Penetrating sex is usually permitted after 6 to 7 weeks.

Non scalpel vasectomy

Good afternoon Shannon,

Perhaps you mean a “no scalpel vasectomy”.  Yes, we can do that.  Please consider that a “non-scalpel” vasectomy is a promotional term, in that local anesthesia is applied with a small needle and is used to wet and soften the skin as well.  Then a pointed scissors with a spreading technique is used to “poke a hole” or tear the tissue down to the vas.  So, something sharp is used after all.

Our fee is 250 for consultation 500 for the procedure.

Closing a “dorsal slit”

I would like inquire about a dorsal slit reversal. Is it possible? if it is, will the outcome be just like an intact penis with all the foreskin restored?  Please give details regarding the costs of operation and recovery period.  Thanks in advance.   Matthew.

Good morning Matthew,

As you may know, a dorsal slit is a longitudinal release of the foreskin on the top side which opens the phimotic ring and usually results in a waddle of skin under the head and distal shaft.  Not that pretty, but often convenient when the patient may not be a suitable candidate for circumcision, such as  a gent with a severe penile infection.   A circumcision could be done secondarily.

Thank you for your interest in what we do.  Several thought come to mind.  Reversal is possible with some caveats.

A dorsal slit is usually done by a crushing clamp technique to reduce bleeding followed by an incision over the crushed area.   The edges are then oversewn to prevent delayed bleeding.  This reduces the amount of foreskin available to reconstruct unless you were very generously endowed to begin with.

Normal for men is a preputial ring at the end of the foreskin which upon reconstruction is the weak point and could lead to tearing with erections.  So while an attempt can be made to do this, there is no guarantee everything will be smooth sailing afterwards.

Fee for reconstruction 1500.    You will need to be on an anti-erection medication for 6 weeks.
