Circed, but with wrinkles on the distal shaft

Had a circ last year in Baltimore. How do you explain some wrinkles I have in
my distal mucosal, or inner skin after circumcision. I thought they would go
away.

Hi Lex,

A circumcision is more than just an approximation of 2 fresh incisions. If the doctor doesn’t like what he sees or knowing how it’s going to look before he’s finished, it would not be very cosmetic to ignore that.

Underneath the loose skin of the penis are tissue septa or strands that limit the amount of stretching penile skin will undergo. On the soles of the feet, the septa are very short, somewhat looser on the palm, still looser on the back of the hand and much looser on the penis.

However, because of chronic inflammation the septa may yield just a little and so when the penile skin is on the stretch, they still are seen, creating little wrinkles.

The way we handle this is to undermine the septa and allow them skin to heal over the dartos fascia without wrinkles. If we see there is a potential for a penile scrotal web, we also free up the skin at the base of the penis on the underside, so it settles better. Please understand that this is our intention and no urologist can guarantee a perfect result, but we try to get as close as we can. We might be a little fussier, and that is why it is important to leave your dressing on for a good week, to immobilize the skin so it will heal with minimal scarring. 

For excesive wrinkled skin we may often removed a strip on the underside and close in the midline to provide a smoother look.

For this reason we prefer to have our patients watch work in progress with a hand held mirror to be sure, they are happy before the dressing is applied.

Zoon’s balanitis

To the Greeks, the glans looks like an acorn, and “balanos” is the Greek word for acorn.   -itis means inflammation of.  So balanitis is an inflammation of the head of the penis and is usually red secondary to inflammation.  Cancer may also present with reddened skin.
 
 
ZOON’S BALANITIS (PLASMA CELL BALANITIS)

This rare form of balanitis was described in 1952. The aetiology is unknown but it has been postulated that the condition may represent a chronic irritant mucositis.

Incidence

Zoon’s balanitis occurs mainly in middle-aged and older men. It is almost exclusively a disease of uncircumcised men. The incidence is unknown but of over 250 consecutive men presenting to our male genital dermatosis clinic, 9% were diagnosed with Zoon’s balanitis. 

Clinical history and examination

Many patients may be unaware of symptoms. Staining of underclothes with blood and discharge may be an incidental fi nding. Occasionally soreness and itching may occur but presentation most often comes about after the patient notices the red appearance of the glans penis.

Solitary, shiny or glistening erythematous plaques may be seen on the glans penis. There is often a corresponding lesion on the prepuce resembling an ink blot pattern. Lesions may have a ‘cayenne pepper’ appearance due to pinpoint purpura. 

Diagnosis 

Diagnosis may be confused with other forms of balanitis, such as fixed drug eruptions, erosive lichen planus,  psoriasis, seborrhoeic eczema and pre-malignant lesions such as Erythroplasia of Queryat (

Skin biopsy is usually necessary to confirm diagnosis. A dense dermal infi ltrate of plasma cells on histology is characteristic. Secondary infection is very common in these patients and microbiological swabs may demonstrate growths of anaerobic bacteria, coliforms and streptococci.

Treatment

Zoon’s balanitis may be very resistant to treatment. Topical agents are standard

fi rst-line therapy. Corticosteroids, antibacterial agents and antifungal agents have all been used with some success. Occasionally, patients respond to hygiene measures alone, particularly if their previous level of hygiene was poor. The definitive treatment is circumcision which is almost always curative. The CO2 laser has also been used with some success.

Is adult male circumcision safe for someone in their forties or late stage in my life? Are there any disadvantages of having an adult circumcision now verse at birth?

Thank you for your interest in what we do.  Your penis is one of the most important functional and cosmetic features of your body. 

Today we did 2 circumcisions… one on a physician from Massachusetts he was about 62, and the other an electrical contractor age 56.  Both did well and had easy, painless procedures.  I’ll be seeing both tomorrow, even though it’s the weekend.

The disadvantage of having circumcision done as an adult is the postoperative course is more involved.  The penis is larger and of course more vascular.  2 days of bed rest is the best guarantee you’ll have minimal discomfort and a smooth convalescence.

Ops, a 1/4″ opening after circumcision, will it go away?

I had a Circ on 4/12/2011, the disolving sutures disolved after a week and a half, there is a opening a 1/4 inch on top and about 1/2 in on the right side its been open for the last month, my surgeon says it will close up, but after a month of being open I dont know if it will close up. do you think it will close up?

Man with non-functioning penile implant has a foreskin problem that is more pressing

 I had a inflatable implant Put in in 1980. It has failed. I  am having problems. The foreskin can no longer retract and I am having a hard time keeping clean. Also no sex. Can the implant be removed or replaced? Will I need to be circumcised? I am 65. my urologist suggest removing the old implant and splitting the foreskin and no sex. Your suggestions please. I do not think he wants to fool with me.

I would consider a circumcision first. Then wait 3 to 4 months for adequate wound healing, then only if you wish, have your  implant replaced. To have done well with a single implant  for 31 years is exceptional. You do have a claim to fame.

Fordyce spots

Hi Dr. Reed,

We spoke in August of last year regarding the tract marks that were left from my circumcision. Please read our emails below for a quick refresher.

I took your advice and went to see a dermatologist and explained the situation in its  full entirety. He gave me a cream called Dermovate  which is a steroid cream that I have to apply nightly. I don’t think this will help as it will not tackle the fact that the skin healed around the stitches creating tiny holes that seem to contain sebum sometimes. It seems to me that the only other way would be to excise the skin from the area.

How much would this procedure cost and how many days would I need to be in Miami for?

Also I have tiny Fordyce Spots on the shaft of my penis and would like them removed. I know they are not of any concern but for me I hate them. I have researched this tremendously and it seems the only method to tackle Fordyce Spots with a hyfrecator.   Do you do hyfrecatio treatments and if not can you recommend anyone to Hyfrecator these tiny Fordyce Spots.

These issues are affecting my sex life and as a young male who loves women, I really want them to get sorted.

Good morning Dillon,

Thank you for your update. Most likely we can do both if the Fordyce spots are not too extensive or clustered so that hyfrecation will leave a noticeable scar on the skin. As you know Fordyce spots are small sebaceous cysts unassociated with hair follicles.

Out fee for both is 1800 to include office consultation, local anesthesia, use of the operating room, and my followup care.

Plan on staying either at Daddy O’s Hotel or the Baltic as I will see you there daily for 2 post-operative days.

Do you induce an erection during surgery to be sure, it will not be too tight

I am terribly sorry for the delay of scheduling my appointment. I was caught up with some personal matters. In either case, I have been doing a little more research on peoples adult circumcision experiences and I have a very specific question regarding style and procedure.

Do you operate on the penis while it is erect to ensure the look is aesthetically pleasing with a full erection, or do you operate on it while it is flaccid? Is it bad practice to operate on a penis while it is erect?

My concern is that my penis is very small when flaccid (1.5 inches) but about 5.5-6inches when erect. The other cosmetic urologist I spoke to said he would not induce an erection neither before or during the procedure (indicating it was unacceptable practice) and he will estimate how much skin to remove only while flaccid with some manual traction to simulate an erection. This makes me worry too much skin will be removed without knowing how much the penis expands on an erection and as a result will cause my penis to become shorter in length.

What is your approach to this? Is operating on an erect penis truly more risky than while it is flaccid? How would we ensure that only enough skin is removed to maintain a good look when fully erect?

Good morning Pattash,

While we do not operate on the penis when erect, we do put the penis on traction to simulate erect length.

Inducing an erection with medication leads to enhanced blood flow and may cause more bruising and blood loss.  Of course we could inject your penis with sterile saline and place a tourniquet at the base as we do for Peyronies’s patients, but this is normally not done. 

If you wish, we could induce an erection in the examination room and pre-mark your penis. Then we could reverse the erection in the operating room.  So there are many ways we can accommodate you.

More importantly is that you like the result when your penis is flaccid.  For every patient we see that is unhappy with their penis erect post circumcision by another doctor, we see 20 times as many patients unhappy with the appearance when flaccid.  For example,  loose skin rolling over the corona or rim of the glans.

That too much skin will be removed is a good hypothetical consideration, but in fact has never happened in our practice.

Does any insurance pay part of this procedure, I have Blue Cross Blue Shield.

Thank you for your interest in what we do.  While the good news is that most insurance companies do pay for circumcision and are fairly liberal,
although they would all insist on a medically valid reason, the bad news is they allow about 200 to 300 for the procedure, and this is why
some doctors tend not to exercise the degree of attention we do.
