Want Adult Circ with Gomco clamp, want to see that brown line

Good afternoon Dr. Reed,

I need an adult circumcision for conversion to Judaism. After some research I decided on a gomco for appearance/aesthetic reasons. I cant find anyone in Boston who will do it on adults even though it is made in sizes for adults. 1) Will Dr. Reed do a gomco ? 2) If not, is it possible to get that brown scar line another way? I just dont want alot of skin ripped off (sleeve resection) and I don’t want a fine white line either. Thanks for the info.  Melvin

Thank you for your interest in what we do.  Called and left a detail message.  Please avoid the Gomco.  Babies have years for erections to shift and adjust skin to bring the general appearance into expected compliance.  Adults more or less stay put where the doctor left them.  Using a Gomco clamp and bell is a somewhat blind procedure and you’ll forfeit a lot of control.

For a brown scar, consider micropigmentation, 232 skin color choices.  On adults we have seen wonders these artists perform.

Generally speaking the penis does not appreciate simultaneous surgery with a few exceptions.

With AlloDerm, we can say quite conservatively you can anticipate a 3/4″ gain in circumference if you have not already had penile girth enhancement which is permanent as far as we know having followed patients for 8 years or so who have had AlloDerm. 
 
Most patients will do a lot better than that.   AlloDerm patients must be on strict bed rest with the exception of getting up to use the commode or eating in their room for 4 days.  Surgery is the zero day, and we certainly recommend you stay at the Baltic Hotel (877 622 5842) so I can see you daily as your penis will continue to swell for several days, and your dressing will be replaced often to accommodate a larger sized penis.  Mention my name for a discount.

AlloDerm is like particle board, when immersed in body fluids, it swells and swells for several days.  This spreading of the collagen fibers is helpful to permit ingrowth of a capillary bed and new cells of your own making.
 
Whereas AlloDerm comes from a cadaver, BellaDerm comes from a live donor.  There are many subtle differences which require telephone explanation.  I have not seen any communicable disease emerge with either product.  AlloDerm is packaged dry and swells like particle board.  BellaDerm is pre-packaged in solution and what you see after surgery is more or less what you get.

Early mobilization  is the great enemy of good graft take.   Again plan on staying in your room, stocking up on food or sending out for food.  Good Italian, Greek and Chinese restaurants abound with delivery service only a telephone call away..  I’ve have been known to go shopping for some patients at Publix.  You won’t starve.

If you have AlloDerm surgery on Tuesday, you could be back in an office environment on the following Monday.  If you do manual labor you may wish to wait a good week after surgery and avoid trauma to your genitals.

Just circumcised, not what I expected

Dear Doctor Reed I was circumcised on 2/18/11 in CT and I know it didn’t come out right. My adult penis has averaged out about 1 1/4′ long when flaccid and about 5 3/4′ long when erect. It also used to point slightly up before the CC but it now has a hard time doing just that. Now my penis isn’t the same. The hairline that was against the base of the shaft is now midway up in the shaft when my penis is erect. I know the skin is holding back my erections because not only does it not feel like it but there is also a slight bulge near the base of the shaft whereas before the penis was wide at top and narrowed down toward pelvic bone. The scrotum is also really close to the shaft, weighing down my penis and thus changing how it points up during an erection. I think it can be fixed if I am re-circumcised and the skin is sewn farther away from the coronal ridge but again I am not sure. There is also a small piece of left over frenulum right under the meatus (urethra opening) and well all of this is depressing.   Manny

Tight Circumcisions, Part 2

So I’ve read that prior to surgery, the penis is stretched to find the correct place to make the marks where the cuts will be made. Does stretching really duplicate how the foreskin is on the erect penis? Sounds a bit hit or miss to me….. those of you who have had it done, please chime in.     Indian man

Dear Sir,

I am not against tight circumcisions. I am against overly tight circumcisions which draws pubic hair onto the shaft, the scrotum onto the underside of the penis and loss of the pubic penile angle when the erection tents (lifts) this skin like a hypotenuse of a triangle.

Many of these patients will wind up in an attorney’s office. Yes, if a doctor makes a somewhat overly tight circumcision on a young man, perhaps his erections may, may stretch the skin out. Less likely on a middle age man.

Patients may ask doctors to do things that predictably will not work out, and a smart doctor needs to say “no”.

Tight Circumcisions, part 1

 I am curious why is it a bad thing to have a tight circumcision. I mean won?t the skin on the shaft stretch and loosen up to accommodate adequately the erection?

Hi Bruce,
We do the circumcision with the penis on stretch so not to make it too tight. It is worse to make it too tight than too loose as pubic hair can be drawn onto the shaft and this may also increase the potential for a penile-scrotal web.  Before the operation is concluded, the penis is taken off stretch and the patient and our surgical team have a chance to review. The patient with a hand held mirror. We are not finished until we obtain the patient’s approval.

The patient is also pre-marked for his approval, so this is not hardly hit or miss. We tell every patient this is your circumcision.

Dr. Reed,

Someone referred to you from the yahoo.com website. I was circumcised last April and my surgeon used dissolving stitches. There some raised bumps where the stitches were. There was tunnels but they’ve gone away.  

I’ve been putting vitamin E on it cause I heard that helps heal scars. I just want the raised skin gone. Even if its a little discolored, just no raised skin. I didn’t start treating the scars till six months after surgery(roughly two months ago).

Someone told me it can take up to 18 months to completely heal. I just wanted to know if this is permanent or if it should heal or if there’s anything more I can do?

If you can’t help me I understand I just don’t have health coverage anymore so I can’t ask my doctor. I got the circumcision done cause I was really insecure about it and the skin was WAY too tight.

Hi Donald,

These raised bumps may be chronically inflamed areas that are the remnants of suture tunnels.  Best remedy is removal together with curettage or delicate knife and closing with ultra fine fast absorbing suture and keep the steri-strips on for 3 or more weeks.

There are also non-surgical ways to loosen your overly tight skin.  Will discuss during your consultation.

Circumcision revision requested, looking for assurances this time

I would like a revision with glans completely exposed. However, I am hopeful this is possible with some ‘movement’ left in the shaft – worried about too much tightness but currently dissatisfied with too sloppy of skin overhang. Is this a common concern? Can a balance be achieved?  Melodious Mel

Phimosis, possible BXO

I’m a 58 yr old male. I’m having problems urinating properly due to the fact that the foreskin on the tip of my penus has recently reduced itself in size preventing my glan from being exposed. All this started when I noticed my foreskin dry with small cracks which by using vaseline I would normalized this situation immediately in previous occasions, but this time around it seems like it took another turn and decided to retract my frontal foreskin. Is this phomosis? What do you recommend? Its quite urgent, please advice. Thanks!

Adult circumcision, more from Antonio, four options

Hi Dr Reed and thank you for getting back to me and with swiftness at that. I appreciate you answering the questions I had about the grant and providing insight about other urologists.
Circumcision is something I wanted done most of my life but because of embarresment and money that has never happend. I find myself in a position now where I can make it happen, though finacially it may be a stupid decision, personally and psychologically I believe it is the best decision.
I have looked over the pics from your patient and have made some observations. From the knowledge I have gained from researching this topic, my guess is that this is a high and tight circumcision. From looking at numerious pictures, I have been pushed away from the high and tight style. I believe this type of cut is very noticable. Even in your patient, the frontmost part of the penis is distinctevly different in color than the naturally looking back end. It is for this reason that I have been leaning toward a low and tight cut. I believe this style doesnt show as much difference in skin tones and hides the scar much better. I’m sure your professional guiadance and advice can better educate me on the different styles making me more comfortable with my choice.
I will be using some extra school money that I will be recieving in late January to pay for this procedure and I hope to have the surgery done a.s.a.p once recieving my money. For some reason I feel you are the right choice for me. I was born in North Miami and raised in Hollywood. Most of my family still lives in the area so it will be just another reason to come down and hang with the fam.
Here are a couple questions that I still have.. Since I will be getting my money Jan 22nd what is the soonest I could have the operation done? From what I read the total cost should be 1750. Is this the exact number I will spend if getting this procedure done by you? lastly, and this is a silly question, would I be allowed to listen to my ipod while you are operating?
I cant wait to hear from you!

A circumcision is probably the highest cosmetic calling given to a urologist.

There are four parameters that do not conflict with each other, and you can choose.  Low or high, tight or loose, remove or do not remove the frenulum, and obliquity of cut.  That is parallel to the corona or perpendicular to the long axis of the shaft.

1750 covers all except medications, perhaps 20 dollars.  Publix gives you Keflex free of charge.

After January 22, 2011 the earliest available date would be January 31, 2011. 

We do recommend you stay overnight at a nearby motel (the Baltic is the most economical) and I will see you there the following morning.   Mention my name for a discount.

First, thank you for getting back to me.  I was circumcised at birth. I have developed some scars underneath my corona that I would like to have removed. Additionally, I would like a tighter cut, maybe with my frenulum removed. Second, I don’t have any real problems with my penis or circumcision. I just want some cosmetic changes. Third, I have a few questions, if you don’t mind and have the time.

1. What are the “four parameters” you respect?
2. “Photos sent and received (unspoken comment ‘disaster zone’)” You mean, the pictures of the penises that you circumcised were ‘disaster zones’ before you circumcised them?
3. I’d never heard of a Japanese circumcision until I opened that document. What is it? What are the benefits?

Thank you for your thoughtful questions which are answered in the order
posed…

1. What are the “four parameters” you respect? 

Firstly, I confer with the patient to find out exactly what type of result he is looking for: tight or loose, high or low, removal of the frenulum or preservation of the frenulum, and whether the cut be parallel to the corona (oblique) or perpendicular the shaft.  Secondly, only fine plastic surgery suture is used to create perfect alignment.  The patient has access to a hand held mirror in the operating room  to be sure the circumcision is being done according to his specifications.

2. “Photos sent and received (unspoken comment ‘disaster zone’)” You mean, the pictures of the penises that you circumcised were ‘disaster zones’ before you circumcised them?  

Thank you, as this is a little unclear and could even be construed that I was initially the operating surgeon.   This a letter from a patient who had been circumcised elsewhere and wasn’t happy with the result.  He sent me photos and later had a revision with me.

3. I’d never heard of a Japanese circumcision until I opened that document.  What is it? What are the benefits?

Please avoid this as the cut is made towards the base of the pubis and all of the foreskin is saved.  Problem is this results in a lot of swelling of the shaft which may take months to resolve.    I am not saying that this is the way all circumcisions are performed in Japan, but this is the appelation was given to me by the patient which is also described on the web.
