Was my circumcision done by a trainee?

Hi Dr Reed i have been looking at some of the excellent work you do there ,and what i have seen has made me want to come to you ,if you can help me that is ? i am a 37 year old from australia I had a circumcision about 2 month`s  ago, the doctor that did it is ment to have 30 year`s exp of doing it under his belt , so i thought that i would be in good hand`s , the day after the surgery i was looking at the job he did ,as i was i noticed how everything looked uneven , 3
weeks later i went back to see the doctor as the swelling on one side had not gone down he said it looks fine and that the skin would shrink back down ,and i was worryed about the way it was healing with the skin bulging over on each side he did`nt put a pressure bandidge on it which a friend of myn found strange , and that it looks almost like the top of a castel as i think they used a thick grade stitches .On the left side of the shaft from the head to the scar line it is
12 mm and on the right is 32 mm from head to scar line and thats how it was when i first looked at it and thats how it stayed and the scar is not at all straight far from it,it has heald up looking ugly.  On the left side of the shaft
the skin did`nt fuse correctly and it overlap`s and it also overlap`s on the right  ,i was informed it took 15 minutes for the surgery and then they must have stapled me up ,sure look`s like it i think a trainee did it myself the 3 blind
mice, do you think there is a way i can make them pay for it to be fixed ? I would like to know a little more about the girth enhancement tech you do i have read some of whats on your web site and mynewsize.com i have tryed to enhance my girth for year`s trying all sort`s of stuff nothing has so far worked pills, rubs ,ointments, everything but surgery i like what i have seen on here and of all the was it can be done which do you think is best and the one you
would have done if you was going to do it yourself?also would it be better to get that done at the same time as a revision and could you maybe give me a price on that please sir . can you help me Dr Reed or do you know of someone

Thank you for your interest in what we do. Yes, we do men in your age group every week.

We have performed over 6000 cosmetic circumcisions in the past 35 years and usually do several cases every week.

We do ask that you stay for one, ideally 2 days at the Baltic Hotel (advanced tourist class) or Daddy’s O’s (more posh) and I will see you there daily.

You could return to an office environment in about 4 days after circumcision. Our 2 layered wrap of soft conforming gauze and Coban will provide comfort and reduce swelling.

We have done sons of physicians, medical students and residents, brothers, religious circumcisions, men who have not seen the heads of their penis for 20 years! This is your circumcision and we do it the way you want with respect to
4 parameters.

Please don’t be like the patient who writes: “Dr. Reed I just got circumcised two days ago and my result was not what i expected. I was wondering when i recover can i get a consultation with you.” Photos sent and received (unspoken
comment “disaster zone”)

OK Lionel, here’s my take and I have a professional responsibility not to be
castigating..

Could have been a lot better. Aside from every critique you mention, of which I would agree, widely placed sutures lead to a lot of puckering which is evident on some of your photos.

Circumcisions are traditionally not well paid for at least in the United States, because conventional wisdom (wrong) is a urologist could run through this procedure in 15 minutes.

When I was a resident, I saw a urologist ask a nurse in the operating room with a prominent clock on the wall…”What time is it.” and the nurse said with some askance “Why doctor it is 8:15.”. Fifteen minutes later he completed the circumcision and the urologist said again, “nurse, what time is it, and she dutifully looked at the clock and complied, “why doctor it is 8:30.” He then said, “Do you mean I just did a circumcision in 15 minutes?” When I look down at the poor patient I thought to myself, it looked like it had been done in 15 minutes.

Lionel, in our practice it takes about 55 minutes. Penile skin does not come cheap.

The unevenness may resolve only somewhat as the swelling subsides, but I would be hard pressed to say this will self-correct. Also there is an area where the penile skin is inverted.

Suggestion, wait a good 3 to 4 months after your circumcision for everything to heal as well as it is going to. If continued improvement is slowly noted, wait even longer, then most likely you will come to revision. We do not maintain a directory of a handful of urologists in Australia who are cosmetically minded (there must be some).  Or see me. In fact we see Australians here quite regularly,

Residual underside edema status post circumcision eventually clears

good morning dr reed . i underwent a circumcision procedure under your care in sept 2007. subsequently i had to come back and have a revision to tighten up some sutures on the underside of the shaft. during my first procedure i also had a partial rhinoplasty done by a friend of yours dr ary krau. after the first circumcision  procedure i had quite a lot of swelling which subsided quite slowly. after the revision the swelling was even worse and i sent you photos and you explained that my lymphatic drainage was being compromised and further surgery could alleviate to problem but would be completely elective. i was unpleased with your response and suggested that i wouldnt trust you to perform another surgery on me. however in the past year i went to see a specialist ENT surgeon who informed me that i had a badly deviated nasal septum to the right side and as the previous rhinoplasty  only involved the external nasal bones i believe my ability to heal was being impaired considerably. hypertension being the main issue. so i would like to apologise to you for suggesting that my post operative swelling and lymphatic drainage were because of your dereliction. i no longer have any issues with swelling of the shaft and im very pleased with the final result. thanks for the great job and dedication. best of luck . c. fitzpatrick

Thank you for your followup letter.

Tincture of time often helps a lot with wound healing if the tissues were properly brought together in the first place.

Nature seems to understand what was intended and expected of it, and eventually (usually) complies.

It was great to hear your voice again and your always optimistic enthusiasm
 
Here’s what my experience has been:
 
As you recall I had a lot of outstanding induration from my initial circumcision all along the incision line attributed to focal inflammatory responses to Suture material (4.0 chromic) which can occasionally happen with absorbable Suture Specifically the induration was pronounced near the right and left of the ventral midline close to the frenulum area which you revised The healing on the right side of the frenulum area has gone really well and I’m surprised that the quality of skin here is just like natural untouched skin that has never been operated on.
                                                                                                                                                                                                                                                         
There’s absolutely no outstanding induration in the area and the skin can be pinched to verify this and the quality of sensation here is exquisite.                                                                                                                                                                                                 
I have to applaud you for this result and your expertise with tissue handling is second to none You’re one of the greatest Cosmetic Urologist’ of our time I would also comment that offering patients a choice of absorbable vs Non-absorbable Suture like 5.0 Nylon maybe a good idea for those seeking the highest level of cosmesis in an Adult Circumcision if having them removed within 2 weeks is feasible for the patient.                                                                                                                                                                                                                                                                  
The advantage here is vastly reduced potential of focal inflammatory responses to suture material as a Non-Absorbable Suture like 5.0 Nylon will most likely not react with the skin

Recurrent micro-tears, an indication for adult circumcision

 I am a 39 year old, never circumcised. I want to get a procedure done, as I am experiencing frequent micro-tears on the skin after the sexual activity and some pain, also possible erectal/sensitivity issues. My girlfriend is suggesting I should get it done. What do I need to get prior to the procedure, do I need a referral from my regular doctor? Also, do you know if my insurance (blue cross blue shield) will cover this procedure, as I need it done for medical, not cosmetic reasons? Any advice will be appreciated, Thank you!    Vladimir
