Unsightly pubic scar from phalloplasty, what to do

Dear Dr. Reed,

Had a combined penile procedure which resulted in some loss of length, and in my pubis, a divot is seen along with a spreading scar extending onto my scrotum.  Plus there’s a bald spot in my pubic hair line. 
Is this something you can correct?

Hi Ken,

We see these scars almost every other month at the office and the reason is distal based flaps usually do not heal well, unless many precautions are taken.  The reason is the blood supply which starts out in the heart is flows down (that  part is OK) but is then turned around and expected to go up or make a U-turn. 
In pre-operative anatomy, blood flow is normally not like that.

At any rate please look at correction of such a case.

I’m already scheduled…
So from what I read I understand that perhaps it is better for me to go low and tight, since you say you’d only recommend to keep about 1″ to 1.5″ of the inner foreskin there.

What about weight lifting? How long should I wait before I get back to the gym?  I don’t do competition kind of stuff, just work out to stay fit and healthy but I do max out on my last sets of each exercise. I’m very concerned with the type of circ I get because obviously I want to get the best results while not having to do a re-circ.

Hi Jordan,

In that during weight lifting there is a tendency for Valsalva (holding your breath and bearing down) which increases venous presure,  I would propose you wait 2 weeks and of course be mindful about the potential for straddle injuries or pressure in the area for a good 8 weeks.

Getting circumcised soon

Hi all,

I wanted to ask a few questions before I get my circumcision done. I’ll get straight to the point…

What kind of circumcision is more popular and why?

Should I get high and tight, low and tight? I am a “grower”. My penis goes from about 4.5″ flaccid to between 8″ and 9″ erect. So my point here is that I do want a very hard erection, but I’m worried about whether I’ll have enough skin to cover such growth.

Should I remove inner or outer skin? I am extremely sensitive in that area so I don’t care if I lose a little Frenulum removed or not?

Basically I want the whole glan exposed when flaccid, and want a very hard erection, but I don’t know whether to go moderate and tight or high and tight?  I hope I am using the right terms here, if not please correct me.

Hi Jordan,

We offer the patient four options: A. to remove or not to remove the frenulum; B. tight or loose or moderate; C. high, medium or low; D. making the cut oblique (parallel to the corona of the glans), or perpendicular to the shaft.  Aside from the fact that high circumcisions take longer for the swelling of distal shaft skin to subside, and if your frenulum when erect is pulling your head down, there are no right or wrong answers.  I think we would all agree that a circumcision which is overly tight and results in lifting your pubic skin when erect, or drawing scrotum onto the penile shaft is not what you had in mind.
 
The inner skin of course is the portal for sexually transmitted diseases such as HIV, so personally I would not encourage you to have more than and inch to 1 1/2″ of that left.
 
Studies have shown that despite some individual variation, on average there is no difference in sensitivity of circumcised vs. uncircumcised men and certainly mean ejaculatory latency times are similar.  Or saying this another way, we see heightened sexual excitement (read: pre-mature ejaculation) as frequently in circumcised as uncircumcised men. 

Bear in mind whoever does your circumcision, a circumcision is perhaps the highest cosmetic calling for most urologists, and you don’t wish to place one of the important cosmetic and functional features of your body in the hands of the occasional operator.

If you having an elective procedure, every risk threatening
aspect of your health should be brought under good control.

Many patients have high blood pressure (HBP), and your doctor should
not let you walk around like that.

If you have a bout on unexpected hypertension before surgery perhaps
related to being overly anxious we will medicate you to keep your blood
pressure in a more normal range.  High blood pressure is not good
for your heart, brain, or kidneys, and when undergoing surgery, could
result in more bleeding and oozing after surgery.
Patients with a history of HPB and on medications to control it,
undergo surgery every day.  Not to worry.

Hello Dr. Reed,

I hope this email finds you in good spirits.

Yes you were right. I needed to go to a qualified doctor to get my stitches removed. And I did. I went to your brother and I have to say he did an amazing job. it was painful but over with in 10 minutes. I am most surprised as to how fast my frenulum area has healed. I have to say it has been more than a week since the stitches came out, and I have healed really well, except for two tiny spots that keep spotting randomly where the stitches were. They are very close to each other and there is no big skin separation and i was wondering if I could put a steri-strip on them? Its because this i dont have to keep applying light dressing all the time during when i am work. That is quite irritating but all in all its pretty good. I have a little hardening of the skin where the stitches were and some peeling but nothing that aveeno moisturizer is not taking care of. I did try Vitamin E oil but it was very itchy so i didnt use it a lot, but continued to stick with aveeno.

I just wanted to thank you and Anne. You made this procedure seem like a walk in the park. I am sure you have lot of patients who will refer you but if you ever need someone in NYC or any other place who is considering circumcision but wants to know about patient experience please do NOT hesitate to give them my email or cell phone number.

Impact of foreskin being able to slide easily back and forth on circumcision (so much the better)

I am 21 years old now and I would like to be circumcised. I have no problems with the foreskin as far as sliding it back and forth. No pain or anything. I would like to know what would be the most common procedure for a male my age? Would the foreskin be cut from the tip? I have seen procedures as to where the penis was cut in the middle of the shaft of an 18 yr old male. Once he said it healed, it didn’t look much different.   Sean

Good morning Sean,

Thank you for your interest in what we do.  Given that your skin slides easily, you could have a low, medium or high cut, although we recommend a medium cut.  Tightness is another issue and we can operate to suit within reason.  We are accustomed to accepting a lot of direction from patients, after all this is your circumcision, and the best way to go about this is through pre-marking while you are on the exam (not operating room)
table.

If your insurance company pays us our fee in full, then of course we accept your insurance.  If not, we may need to do you in a hospital setting which is a little more involved for both of us.  Anne our amiable office manage could best advise.

We have performed over 6000 cosmetic circumcisions in the past 35 years and usually do several cases every week.

We have done sons of physicians, brothers, religious circumcisions, men who have not seen the heads of their penis for 20 years!  This is your circumcision and we do it the way you want with respect
to 4 parameters.

Combining length and girth, primrose path to disaster

Which procedure do you recommend for girth enhancement? Which one feels the most like pre op flesh?  Would girth enhancement alone prevent the penis from shrinking and do you gain any length from the girth procedure?  Do you use any implants in the lengthening procedure to prevent retraction?  

Can any type of girth procedure be done together with lengthening?
Would I be able to drive home 3 days after the procedure (10 hr drive)?   Calvin

Thank you for your interest in what we do.

Either AlloDerm or BellaDerm would be acceptable and in our hands produce similar results.  No sir, do no combine length and girth enhancement as the results for length are lackluster at best and often disastrous.  We have patients coming to our office every month who have had “get-there-itis” and rue the day.   You cannot drive home or go to the mall after the procedure.  Right to bed at a nearby motel and bed rest for 4 days.  Yes, sir. 

You want a beautiful result or do you want your penis to have more wrinkles than a stewed prune?

Girth enhancement will not prevent the penis from flaccid contracture as the material is soft (like a wet chamois).

We tried implants for length about 12 years or so ago, and desisted because of the overly high incidence of extrusion of the silicone strips.  We have also heard repeatedly that other physician(s) who employ this technique are (in the here and now) having patients with similar experiences.

Not all that glitters is gold.

I am interested in the belloderm procedure. My measurements are 6 x 4.5 what sort of gains would I expect. do the grafts reabsorbe over time. I live in Australia, how would I have to stay in USA for recovery. And how long is recommended to have off work, I work in construction.

Good morning Harnon,

Thank you for your interest in what we do.

The grafts may, (may) reabsorb by about 10% but this is more than offset by replacement with cells of your own making. Please expect some added girth from the bruise effect which will subside, but afterwards the wind up is you should have a noticeable difference in girth.

Please avoid any possibility for genital trauma or straddle injury for a good 6 to 7 weeks. Yes, if caution is exercised you could return to work earlier, perhaps 2 weeks after surgery.

I would like to know a little more about  the girth enhancement  tech you do i have read some of whats on your web site  and mynewsize.com i have tryed to enhance my girth for year`s trying all sort`s of stuff nothing has so far worked pills, rubs ,ointments, everything but surgery i like what i have seen on here  and of all the was it can be done which do you think is best  and the one you would have done if you was going to do it yourself?also  would it be better to get that done at the same time as a revision  and could you maybe give me a price on that please sir . can you help me Dr Reed    or do you know of someone in australia?       i look forward to hearing from you thank you   Dr Reed  i have attached a few  photo`s    yours truely   Lionel

As Reported by The Associated Press:

The California Medical Board has accused an Orange County plastic surgeon of sexual misconduct, including having sex with a patient in the examination room.

Dr. K  faces having his license revoked for two instances cited in the board?s accusation.

According to the documents, in 2009 Knight had sex with a patient while her husband and children waited in the parking lot. When the patient?s husband brought the children inside to use the bathroom, he heard moans from the examination room. There, he discovered his wife having sex with Dr. K.

K, 41, declined to comment Tuesday to the Orange County Register

That being said, pills and creams do not work (thus far).  Traction does for length only, when used under the guidance of a knowledgeable urologist.   I would propose you have your circumcision revised first, as traction which of necessity needs to applied after penile lengthening will rip your suture lines apart.  Also the penis does not appreciate too much simultaneous surgery.

Any verifiable news reports, corrections  or followups would be appreciated
