BellaDerm vs. AlloDerm Phalloplasty for Girth

Saturday, July 31st, 2010

I am thinking about PE surgery and wanted to know the difference between alloderm and belladerm.  What are the differences between the two and which one gives a superior results as far as girth enhancement?

After girth enhancement surgery, with either alloderm or belladerm, can a second girth procedure be done?

How long after lengthening surgery can you start to stretch with thegrip?

Any thoughts on the Dr. E’s silicone implant and the late Dr. Perovic’s bio-scaffold?

How was it possible the patient in example#2 was able to get such a great girth enhancement?

Are you going to post some before and after pictures of patients with glans enhancements?

You are probably aware of the forum, mynewsize.com/forum and the owner/moderator, C. H, had trouble with his 2nd girth surgery. Good or bad, your name is not mentioned all that much on that forum.

Can the $250 consultation fee be applied towards surgery?

Thank you for your time.

Sincerely
Carl

July 27, 2010

Good morning Carl,

Thank you for your interest in what we do.

As consultation (by us) is an independent event, so we do not  co-mingle or reapply fees.  Nor do we try to sell.  We have told many a well hung man he can rest on his laurels and no surgery is needed.

Taking your thoughtful questions in  the order posed…

What are the differences between the two and which one gives a superior results as far as girth enhancement?Personally I do not see any difference in the ultimate result.  AlloDerm comes from a deceased donor, BellaDerm from a live donor (usually tummy tucks with massive amounts of skin removed).  The same corporation that sells BellaDerm also sell deceased donor products as well.

The maximal thicknesses are comparable, but BellaDerm comes in 20% lager widths, so when patients are buying volume and the doctor is charging the same amount for either, BellaDerm provides more volume for the same money.

After girth enhancement surgery, with either AlloDerm or BellaDerm, can a second girth procedure be done?Yes, but I think you’ll be happy enough with one application that you should not have any more girth enhancement surgery.

How long after lengthening surgery can you start to stretch with The Grip?We have our patients doing penis pulling exercises (manually) the day after surgery and it is not painful.  The Grip could be applied about 5 to 6 days after surgery (the zero day).

Any thoughts on the Dr. E’s silicone implant and the late Dr. Perovic’s bio-scaffold?Dr. E claims good experience with the product.  Our early experience with “crescentic” molds despite use of permanent mltiple Prolene sutures showed that as the spermatic cord occasionally lifts the testicle which is a normal thermo-regulatory mechanism, the capsule which tended to fixate to the scrotal wall separated from the testis and some patients reported “empty cup”.  We also did seminal analyses before and after cups and were really taken aback when they dropped to near infertile levels.

Dr. Perovic my dear recently departed friend introduced the scaffold also pioneered by Dr. Kim from Korea.
At first they were impregnated with tissue culture cells of the implanted patient to permit more inward cell growth.

Dr. Atala noted research worker on tissue engineering and past president of the AUA has said if a scaffold is one cm or less in thickness, prior implantation of tissue culture cells growth is not needed.  Ingrowth of cells follows blood supply and capillaries will grow into a one cm. matrix without difficulty.

As for the results, I would suggest you speak to patients who have had this done.  I personally never used this product because we had excellent success with AlloDerm.

How was it possible the patient in example #2 was able to get such a great girth enhancement?Example #2 had 3 layers of AlloDerm put in over 1 1/2 years.  We don’t recommend that, but this was his insistent wish.

Are you going to post some before and after pictures of patients with glans enhancements?

Yes, when we have some mature results.

You are probably aware of the forum, mynewsize.com/forum and the  owner/moderator, C. H, had trouble with his 2nd girth surgery. Good or bad, your name is not mentioned all that much on that forum.

Not aware.

Best wishes,

Circumcision, will it be painful?

Saturday, July 31st, 2010

I’m 27 years old and seriously considering this procedure of circumcision. I just need to know if the procedure is painful & if my insurance will cover it. Thank you Everett

July 25, 2010

Good afternoon Everett,

Thank you for your interest in what we do.  Next to the nose on your face, your penis is one of the most important cosmetic features of your body.

We are contracted providers for Medicare and Blue Cross/Blue Shield (non Blue Options/non HMO) only.  If your insurance policy does not include us in the network of your providers, we will fill out your form for maximal reimbursement to you.  Please confer with your carrier beforehand.

We have performed over 6000 cosmetic circumcisions in the past 35 years and usually do several cases every week.  I can tell you ourright, you will not have any pain during the procedure, and discomfort aftrewards is variable, but for most usually controlled with a few Tylenols.

We do ask that you stay for one, ideally 2 days at the Baltic Hotel (advanced tourist class) on bed rest and I will see you there daily.

You could return to an office environment 3 days after circumcision.  Our 2 layered wrap of soft conforming gauze and Coban will provide comfort and reduce swelling.

Be assured we will show you every courtesy,

With kindest regards,

Harold M. Reed, M.D.

Good morning Robert,

Thank you for your interest in what we do.  What you described is quite common but did not have to be.  A circumcision should be the highest cosmetic calling of a urologist.

We have performed over 6000 cosmetic circumcisions in the past 35 years and quite a few revisions as well, usually doing several cases every week.

Will Blue Cross/Blue Shield cover my circumcision

Saturday, July 31st, 2010

I’m considering an adult circumcision. I would like it done for both cosmetic and medical reasons. The ‘head’ commonly gets inflamed. I have a very comprehensive insurance plan with Blue Cross Blue Shield of Minnesota and am wondering if something like this would be covered. Thanks in advance.  Harvey

July 24, 2010

Good morning Harvey,

Thank you for your interest in what we do.  We do accept Blue Cross/Blue Shield, non HMO, non Blue Options.  Given the small allowance you would be done in a hospital and your stay might be 2 or 3 days longer.

With kindest regards,

Harold M. Reed, M.D.

“Suffering from phimosis”

Saturday, July 31st, 2010

Hi Doctor, D you see patients in the SF Bay area? I am a 40 year old suffering with Phimosis and considering a circ. Are there any cons of getting it done at this age? Thanks, ~Ken

July 24, 2010

Good afternoon Ken,

Thank you for your interest in what we do.  Please see examples of phimotic patients on our web-site.   Regretfully our only office is in greater Miami.  We see patients from all over the world who recognize their penis cosmetically, next to the nose on their face is one of the most important cosmetic areas of the male body.

With kindest regards,

Unhappy camper who just had circumcision

Saturday, July 31st, 2010

Hello Dr. Reed. Well I was recently circumsiced at 44 years old for irritation problems. This surgery was 10 days ago and I’m not too happy with how my penis looks. Theres some skin bunching over my glans. It is only when I’m very flaccid and soft and when I’m sitting. Dr said it will flatten in a couple of months  because the skin is still swallen. I have a small penis when flaccid and with this bunching skin behind and a little over my glans it doesn’t look good. when erected looks very nice and circumsiced and shows a completaly bared glans and the skin stretched. Is it true that after heal is completed and the swallen in gone that the skin will retract completaly when flaccid? If not I’m thinking on have it fixed by you.  Rudolfo.

July 24, 2010

Good morning Rudolfo,

Thank you for your interest in what we do.

We have performed over 6000 cosmetic circumcisions and revisions in the past 35 years and usually do several cases every week.

If when your penis is erect the penile shaft skin could still be tighter without lifting of pubic skin, then there is room for “tightening.”

We do ask that you stay for one, ideally 2 days at the Baltic Hotel (advanced tourist class) and I will see you there daily.

With kindest regards,

Phimosis

Saturday, July 31st, 2010

Hi Dr Reed, I am 37 years old and I am interested in getting my phimotic condition corrected with circumcision. I understand that you use micro sutures used in plastic surgery. Are these self-dissolving sutures? Would I need to revisit you or go to a local urologist to have the sutures removed? Also, what are your normal office hours for me to call your office manager to schedule an appointment with you? Thank you, Arnaud

Hi Arnaud,

Thank you for your interest in what we do.

We have performed over 6000 cosmetic circumcisions in the past 35 years and usually do several cases every week.  Many of our patients have phimosis so we well understand the principles of getting the best result.

Our sutrures while fast absorbing still should be removed 12 days after surgery to prevent permanent cross hatch marks (ghost lines and pin holes).  Please do not let anyone talk you out of having them removed, by saying “they are absorbable, they’ll melt out.”

This should be done by a health care practitioner who has a bright over head light, loupes (eyeglass with little telescopes on  the end) and delicate plastic surgery instruments  like for removing eyelid sutures)  EMLA cream could be applied for 20 minutes beforehand if you are very sensitive.  Some urologists have this equipment in their office if they do delicate surgery, or consider an urgent care center, or an emergency room of a quiet community hospital (not a county “zoo”).

Next to the nose on your face, your penis is a very important cosmetic structure and standards for care should be no different.

We do ask that you stay for one, ideally 2 days at the Baltic Hotel (advanced tourist class) and I will see you there daily.

You could return to an office environment 3 days after circumcision.  Our 2 layered wrap of soft conforming gauze and Coban will provide comfort and reduce swelling.

We have done sons of physicians, medical students and residents, brothers, religious circumcisions, men who have not seen the heads of their penis for 20 years, and next week a father and 3 sons!

This is your circumcision and we do it the way you want with respect to 4 parameters.

Please don’t be like the patient who writes: “Dr. Reed I just got circumcised two days ago and my result was not what i expected.   I was wondering when i recover can i get a consultation with you.”   Photos sent and received (unspoken comment “disaster zone”)

Our office hours are from 9 to 5, Monday through Friday.     Be assured  we will show you every courtesy.

With kindest regards,

Unexpected or unexplained neonatal circumcision

Tuesday, July 6th, 2010

Dr. i have my babe 3 week ago the Dr. he did the circumcision to my babe he did`t explain to me what is the circumcision my question is if the circumcision is revercible or he can have surgery foreskin restoration please help me thank you.  Alicia

July 6, 2010

Good morning Alicia,

Sounds like the circumcision of your son was not properly discussed with you.

Despite your misgivings now, a circumcision does confer many advantages health wise.  Such as avoidance of phimosis, decreased likelihood of urinary tract infections, deceased likelihood of his acquiring HIV and sexually transmitted disease.  It could be reversed later on after adolescence.  There are conservative (non-operative) means to do this, but chances are your son will appreciate being circumcised.  Doctors need to take time to talk to their patients and the family, so that you can make an informed decision.

Sincerely,

Harold M. Reed, M.D.
