Adult circumcision and circumcision revision

Hi Dr Reed,

Some circumcision questions regarding circumcision on a work trip:

a) A while back you mentioned (I think) that you use double suturing for circumcisions? What exactly is it?

b) Would it be possible to use non-dissolvable sutures instead of dissolvable ones, and would I be able to
remove them myself 5 days or so later?

c) Is it possible to erase previous suture marks?

d) What’s your advice regarding exercise?

e) A cirucmcision revision shouldn’t interfere with my ability to perform (non-physical) work should it -
like attending a conference?

f) I would like to be cut “shiny tight”, but would that cause scrotal skin to be pulled forward too much?  How is that prevented?

g) Also, how much would a revision of a circumcision cost? as opposed to a first circumcison?

Taking your thoughtful questions in the order posed.

a) A while back you mentioned (I think) that you use double suturing for circumcisions? What exactly is it?Not familiar with this technique except to say the deep fibroareolar layer needs to be approximated independently at times.

 

b) Would it be possible to use non-dissolvable sutures instead of  dissolvable ones, and would I be able to remove them myself  5 days or so later?Non dissolvable sutures are not recommended in that removing delicate sutures which may be imbedded in the tissues is very difficult even for a doctor without loupes, a bright overhead light, and delicate plastic surgery instruments.  We use Vicryl Rapid hich often will melt out in 12 days or so.  At that time 80% will come out with a gently applied tweezers and the rest of course have to be clipped.

 

c) Is it possible to erase previous suture marks?That is our goal, yes.

 

d) What’s your advice regarding exercise?Easy does it for 6 weeks as incisions take 6 weeks to regain full tensile strength.  You can walk some the next day and go shopping and do upper body exercises.

 

e) A circumcision revision shouldn’t interfere with my ability  to perform (non-physical) work should it – like attending a conference?After 1 day of rest, you should be fine, but do not push yourself.

 

 f) I would like to be cut “shiny tight”, but would that cause scrotal skin to be pulled forward too much?  How is that prevented? Could certainly do this and when we pre mark, you have to participate in that decision.

 

g) Also, how much would a revision of a circumcision cost?  as opposed to a first circumcision?Our fee is the same in that you will require exacting attention to detail and if the incision is made circumferentially, the amount of time is virtually the same, and the number of sutures is of course
