Uneven result, status post infant circumcision

A circumcision is perhaps the highest cosmetic
calling given to a urologist. 

Regretfully we do not maintain a directory of a handful
of urologists who are exceedingly gifted in cosmetic circumcision.

Our patients come from all over the world including
the UK, several each month, and even Kazakhstan
(from a small town Alma-Ata, 150 miles west of the Chinese
border).  One patient said something I’ll never forget…
“my penis is my calling card.”

We do several circumcisions and a few revisions
here every week, as we enjoy a reputation for
excellence and exactitude when it comes to
cosmetically performed adult circumcisions.  We
operate within tolerances of 1/32″ or about 1/2 mm.

Pain is virtually non-existent among our patients.
They smile as they leave the office, and by and
large remain free of discomfort as evidenced by our
followup contact with them.

After seeing so many circumcised men who look very
much like they were done later in life, because
of large suture tracts, and irregularly meeting
skin, I developed perhaps a unique approach to
be sure this did not happen to my patients,
and have found no need to vary over the past 15
years.

Firstly, I confer with the patient to find out
exactly what type of result he is looking for,
tight or loose, no unpigmented skin or some
left, removal of the frenulum or preservation
of the frenulum, etc.  Secondly, only fine
plastic surgery suture is used to create perfect
alignment.

Partial adult circumcision

Thank you for all that information. I was also wondering about types of circumcision; I know that this would be discussed at consultation, but is a “Partial Circumcision” a possibility?

Good morning Jefferson,

A partial is a possibility, of course.  Please keep in mind there is a tremendous amount of swelling that occurs post-operatively and you may need to cleanse with Q-tips for 6 to 8 weeks.  Anytime the inner skin is traumatized the response is swelling as the connective tissues underneath are very loose to permit normal sliding.

Testicular Enhancement

I have a few questions regarding testicular enhancement. I believe my 
testicles are healthy but have lost mass due to testosterone 
replacement therapy. Additionally, they have never really dangled to 
my satisfaction. With this in mind ….
 
•Does the implant procedure itself eventually cause the scrotum to 
stretch and therefore not draw up as radically?
•How much down time after surgery?
•How detectable is it that there is something additional in the ball 
sac?
•What is the price range for such surgery and is it outpatient?

Taking your thoughtful questions in the order posed…

Additionally, they have never really dangled to my satisfaction. With this in mind ….

1.  The testes may be elevated either because the spermatic cord is too tight for various reasons or the scrotum which is a hammock of sorts is not dependent enough and your testes simply have settled down as far as they can go.

2.  Does the implant procedure itself eventually cause the scrotum to  stretch and therefore not draw up as radically?

No although a purse string suture can be placed above, this does not work predictably well.  What will work is a small belt, if you will, made by companies such as http://www.leatherpost.com/static/TC001.html or http://www.leatheretc.com/Merchant2/merchant.mvc?Screen=CTGY&Category_Code=Cock_Rings  Use it with care as you do not want to injure your existing testes.

3. How much down time after surgery? 

Plan on spending 3 to 4 days on bed rest at a near by motel before returning home, and you could be in an office environment in about 2 days after that.

4. How detectable is it that there is something additional in the ball 
sac?
For someone who is intimate with you, only perhaps.  For sheer appearance even up close, probably impossible to tell.
5. What is the price range for such surgery and is it outpatient?

Adult Circumcision, how tight can it be?

Many patients ask for “high and tight.”  Actually we try to discourage the extremes of high and tight, because when there is lifting of the pubic skin or as we say triangulation of the penile-pubic junction when erect, this is way too tight, and for sure the doctor is not off the hook because after some deliberation by the patient he’ll realize that the doctor made it too tight.  We say shiny tight.  Not good.  There is no more pleasure that can be derived from a shiny tight penis than a penis with somewhat mobile penile skin, but in no way comes close to covering the glans. 

Of course no patient wants it that loose it covers the glans in any part.  For that reason, we have hand held mirrors in the operating room and tell patients to be sure they are happy with the laxity or degree of tightness before we close.  We can also put the penis on the stretch to simulate an erection.

Limited Circumcision

Can you remove just the distal portion of the foreskin.

Hi Jim,

Occasionally we get requests from patients who just want a cigar clipping of their phimotic ring so they can be easily retract their foreskin for daily hygiene.  Yes, we do it.  Healing time is slightly longer because the remaining foreskin will swell up, which also happens with a routine adult circumcision, but in that most of it has been removed, the effect is less noticeable.

High or low, which is a better adult circumcision?

Hi Frank,

Consider yourself standing with a flaccid penis.  The closer the incision is to your pubis, the “high”er it is.  The further down or closer to your head (glans) the “low”er it is.

My bias is that there are no advantages either way.  But there are some negatives.  The higher the incision the longer it will take for post-operative edema of the inner skin (non pigmented) to subside.  This could be an additional month or two in extreme situations.  If an incision is too low, we are talking about matching the circumferences, the collar or neck circumference next to your head which is always smaller to a bigger one on the shaft.  To work two uneven circles in together means there will be some puckers or a V-dart in the larger circumference has to be made.  Added scars may be unsightly for some patients.

Moderation always makes sense.

Testicular Implants

Thank you for your reply.  I hope you don’t mind additional questions.
Do any of your patients ever opt to have testicular implants removed after installation?  Even if there is no occurrence of infection, does this procedure ever “go bad” even after some time has elapsed?
 
Do any of your patients have trouble adjusting to testicular implants?  I ask this because it’s hard to imagine having a foreign object in my scrotum.  My testicles are very sensitive because of their small size.  Do implants add to or detract from testicular sensitivity?  (I am not asking about scrotal sensitivity.)  As I am on testosteroone injections, would implants have any effect on how my testicles currently function?  I know that implants would not function in any way like a real testicle.
 
As I live in Arizona, does your clinic offer discounts for air fare or hotel accomodations?  Do you provide shuttle services to/from your cllinic to hotel?

Taking your thoughtful questions in the order posed.

Yes, one patient did and brought to the office two perfectly normal implants.  Could not relate to his complaints and he continues to come to the office and have other procedures done.  So one out of about 200 to 300 cases.  The implants were removed abroad.

2.  Even if there is no occurrence of infection, does this procedure ever “go bad” even after some
time has elapsed? 

Not to my knowledge.
 
3.  Do any of your patients have trouble adjusting to testicular implants? 

Not one.

4.  Do implants add to or detract from testicular sensitivity? 

Neither.  The implants are outside the membrane that surrounds your true testes, so there is no known adverse effect on there function.

5.  As I live in Arizona, does your clinic offer discounts for air fare or hotel accommodations?  Do you provide shuttle services to/from your clinic to hotel?

Compared to other phalloplasty surgeons and considering our experience and results, we find our fees to be reasonable.  Shuttle service is available for about 19 dollars at both Fort Lauderdale Airport FLL (Go-Transportation formerly Share a Ride) and at Miami MIA with SuperShuttle.  Any starter in a kiosk at ground transportation
will help you.

We do recommend you stay at an advanced tourist class hotel, the Baltic Hotel, where the resident owner manager Christine, has shown a great partiality to our patients.  The Hotel is an 8 minute ride from the office and I will be seeing you there after surgery daily for a few days.  Yes, you pay for the hotel usually
about 85 per day.  Gourmet Danish and coffee is served every money and many convenience restaurants of almost every ethnic stripe will deliver.  There is also a small refrigerator in our room and an abundance of supermarkets nearby.  We have had doctors and attorneys, professional people, and people of means stay there.  There were very pleased with the value they got for their money.

We try assiduously to avoid merchandising or selling procedures with package deals.

Yes, paraphimosis is when the foreskin is on the shaft, the head
exposed, and it cannot be easily pulled back over the head.

Phimosis is, as you already know, when the foreskin cannot be
retracted for daily hygiene.

All this is to say, your foreskin ring or phimotic ring, is too tight
and yes I would also recommend circumcision.

Your penis is your calling card. Please have it done
by someone with 5000 case experience who will provide
you with a hand held mirror in the operating room
to be sure it is done precisely as you wish.

Phimosis and Diabetes, Remedy Adult Circumcision

quests: Dr. Reed I can not pull the foreskin back at all. I've been
diagnosed with Diabetes. I'm thinking I may have a case of Phimosis,
could you help? And would this be covered under insurance? If not how

We have seen several hundred cases of phimosis associated
with diabetes and inevitably every patient has been better following
circumcision.
